The Senior Care Coordinator plays a vital role in assessing members’ needs, advocating for essential services, and ensuring members receive ongoing support through outreach, education, and personalized follow-up. By working closely with members, healthcare providers, hospitals, and community organizations, the Senior Care Coordinator helps bridge gaps in care and connects individuals to resources that improve their health, well-being, and quality of life. This role is key to advancing our "LOVE, LEARN, GROW" philosophy, ensuring each member feels valued, empowered, and supported throughout their healthcare journey.

Responsibilities:

    • Member Needs Assessment: Conduct interviews to determine member needs, ensuring each individual receives personalized support tailored to their unique circumstances through outreach or in-person visits.
    • Advocacy: Serve as an advocate for members by connecting them with essential services and resources, helping them navigate social, medical, and community-based programs.
    • Progress Tracking: Monitor and document each member’s progress, ensuring consistent follow-up to address evolving needs and challenges to meet business needs.
    • Member Care Coordination & Reporting: Collect, analyze, and summarize member outcomes and compile statistics, providing accurate and timely reports for Wider Circle leadership to track program effectiveness and outcomes as needed.
    • Collaboration: Work closely with community partners, healthcare providers, hospital staff, and social service organizations to ensure members receive comprehensive care, in line with Wider Circle’s commitment to support its community members.
    • Healthcare Navigation: Support individuals in understanding and navigating the healthcare system, including helping members access appointments, services, and close care gaps.
    • Hospital Visits & Follow-Up: Conduct hospital visits to meet with members, assess immediate needs, provide support during transitions of care, and complete follow-up outreach as needed to ensure continuity of care and access to resources.
    • In-Home Visits: Conduct in-home visits to meet with members that you are unable to reach, review their needs, discuss access to care, and assist with closing care gaps.
    • Housing Assessment: Meet with members to evaluate housing conditions and identify barriers that may impact their health, safety, or access to care.

Physical Demands:

To successfully execute their role, Senior Care Coordinators must be able to perform the following essential functions. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Engagement personnel will be regularly required to:

      • Safely and consistently drive to public places up to 30 miles away from their home
      • Frequently carry up to 30 pounds of supplies
      • Frequently stand and comfortably speak publicly, including projecting their voice in indoor and outdoor spaces in front of groups
      • Routinely sit, stand, and walk to allow the team member to interact with members for the duration of each event effectively
      • Ability to see and hear well enough to interact with members by phone and in-person and respond to member questions
      • Ability to use a computer to view, enter and update information in different systems
      • Effectively communicate with members in person or virtually

Requirements

    • Certification: Hold an active Community Health Worker certification in the state of New Jersey, OR have relevant community outreach or engagement experience and be willing to complete Wider Circle’s Community Health Worker training program upon hire.
      • Bilingual in English and Spanish required (spoken and written).
      • Team Player: Able to work well with others, contribute to a positive environment, and provide leadership when needed.
      • Proactive: Take the lead on community engagement initiatives with minimal direction.
      • Adaptable: Comfortable in a fast-paced, changing environment, and eager to learn and grow.
      • Experience: 3+ years in community outreach, teaching, or facilitation. Healthcare experience is a plus.
      • Education: Associate's degree preferred; 5+ years of relevant experience may be considered instead of a degree.
      • Community Building: Experience creating or supporting a community of 500+ members.
      • Organizational Skills: Able to prioritize tasks, stay organized, and follow through on commitments.
      • Tech Savvy: Comfortable using web-based and app-based systems.
      • Independent Worker: Able to manage tasks and responsibilities in a remote environment with little supervision.
      • Transportation: Reliable transportation for home visits, willing to travel 20-50 miles.
      • Flexible Schedule: Available to work outside regular business hours when needed.
      • Drug-Free Commitment: Willingness to complete pre-employment drug testing and background checks.
