Advanced Practice Provider Palliative Care

The Palliative Care NP is a masters-prepared and certified Nurse Practitioner who has obtained ACLS/BLS certification and is willing to obtain Palliative Care certification within approximately 2 years of employment. The Palliative Care NP is trained to provide care for patients who are living with complex medical conditions and life-limiting illness. It requires the ability to provide care based on physical, psychosocial, spiritual and emotional assessments. The NP is able to effectively communicate with the patient, family and interdisciplinary team to ensure the care being provided is consistent with patient/family goals.
Job Details
Responsibilities :
- Attend interdisciplinary palliative care team meetings
- Membership in the St. Francis Hospital Ethics Committee
- Prepare and maintain accurate patient records, charts and documents to support palliative care practice
- Will assist the patient/family with completing advanced directives and establishing GOC which can potentially reduce the utilization of advanced medical care in the SDU/ICU units
- Order and titrate medications to relieve symptom burden using evidence-based guidelines
- Will educate patients, families and staff regarding the natural history of disease, the patient’s prognosis, what to expect at end of life, and side effects of medications
- Adheres to practices of confidentiality regarding patients, family, staff and organization
- Implement palliative care NP billing standards into daily documentation
- Ensure compliance with legal, regulatory and clinical practices and procedures
- Other duties may be assigned in addition to those listed as necessary
Requirements
- Bachelors of Science in Nursing (BSN) degree
- Master’s Degree in Nursing with specialization as a Nurse Practitioner, and is certified by the ANCC or AANP organizations within 2 years of employment.
- Perform patient assessments.
- Education: Bachelors of Science in Nursing (BSN) degree
- Licensure Current and valid Nurse Practitioner (NP) and Registered Nurse (RN) licenses in the State of New York
- Certification: Basic Life Support (BLS) and Advanced life support (ACLS) required American Heart Association
Posted Salary Range
USD $66.55 - USD $93.81 /Hr.This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
