Our spectrum of home-based services now includes Skilled Home Health Care, Palliative Care, Adult Hospice Care and Pediatric Care. <p><span style="font-size: 12pt;"><strong>This position offers competitive pay, generous benefits, shift differential and a supportive work environment.</strong></span><br><br>We are currently seeking an<strong> RN </strong>to be a part of our<strong> </strong>team. The ideal candidate will provide coverage for After-Hours to cover patient/staffing needs and processes admissions for eligible hospice patients in coordination with Team Manager.</p>

<p>Thursday - Monday.; 4:00pm-12:30am.</p>
<p><span style="text-decoration: underline;"><strong>Location:</strong></span></p>
<p>Marin County with additional coverage to San Francisco & Sonoma Counties as needed<br> <br><strong><u>Essential Duties & Responsibilities include </u></strong>(but are not limited to):</p>
<ul>
<li>Make in-person nursing visits to patients as assigned, or as indicated.</li>
<li>Provide continuing care to patients/families as requested and in compliance with agency policies and procedures and applicable federal and state regulations.</li>
<li>Use the nursing process to assess, plan, intervene and evaluate following patient/family needs and goals.</li>
<li>Provide effective management of symptoms, including prompt escalation to providers or others as indicated.</li>
<li>Coordinate patient/family services and prioritization of needs with the members of the transdisciplinary team or outside providers/services.</li>
</ul>
<p><strong><u>Education/Experience</u>:<br></strong></p>
<ul>
<li>Associate's or Bachelor’s degree in nursing from an accredited School of Nursing</li>
<li>Experience in acute care/ home health and/or hospice setting preferred</li>
</ul>
<p><strong><u>Certificates, Licenses, Registrations</u>:<br></strong></p>
<ul>
<li>Current license as a registered nurse in the State of California.</li>
<li>Public Health Nursing certification highly desirable.</li>
<li>HPNA certification preferred.</li>
</ul>
<p><strong><u>Pay Range:</u></strong><br><em><span class="collapsed-field-text">$55.50 - $69.50 +</span> <strong>after hours/weekend shift differential</strong></em></p>
