The ideal candidate will provide comprehensive care coordination and support to patients with chronic conditions, ensuring they receive optimal care while meeting quality metrics and program goals.</p> <h2>Required Qualifications</h2> <ul> <li>Medical Assistant certification strongly preferred<br>• Minimum 3 years of clinical experience in a healthcare or healthcare office setting<br>• Strong knowledge of chronic conditions and care management<br>• Proficiency in Electronic Health Records (EHR) systems and familiarity with clinical workflows<br>• Excellent communication and interpersonal skills<br>• Experience in patient education and care coordination</li> <li>Strong problem skills and the ability to operate independently<br><br></li> </ul> <h2>Key Responsibilities</h2> <ul> <li>Conduct regular check-ins with the assigned patient panel via telephone<br>• Develop and maintain comprehensive care plans<br>• Coordinate care between healthcare providers and services<br>• Document all patient interactions in EHR system<br>• Monitor and track patient progress and compliance<br>• Identify and address barriers to care<br>• Provide patient education on chronic condition management<br>• Meet monthly performance metrics and quality goals<br><br></li> </ul> <h1>Additional Information</h1> <p><strong>Location: </strong>Remote, preferred in the state Georgia or neighboring states<br><strong>Employment Type: </strong>Full-time<br><strong>Benefits: </strong><em>Competitive salary, comprehensive benefits package including medical, dental, vision, 401(k), paid time off, and professional development opportunities.</em></p><div class="content-conclusion"><p>Aylo Health is an Equal Opportunity Employer</p></div>
