JOB DESCRIPTION TITLE : CARE MANAGEMENT CASE AIDE - MEALS ONLY DEFINITION : Reporting to a Care Manager or a Care Management Supervisor, this position is responsible for the care management of individuals who have declined OPTIONS or Waiver programs and are receiving home delivered meals only.
