<p><span data-contrast="auto">The Care Manager, RN is responsible for conducting initial in-home assessments, orientations for new patients and caregivers, and providing ongoing care management and support. This role combines clinical assessment, care coordination, patient and caregiver education, and quality assurance to ensure safe, effective, and person-centered care.</span><span data-ccp-props="{"201341983":0,"335559739":60,"335559740":276}"> </span></p>
<p><span data-ccp-props="{"201341983":0,"335559739":60,"335559740":276}"> </span><span data-contrast="auto">This role collaborates closely with patients, caregivers, and interdisciplinary care teams to assess needs, develop individualized plans of care, and facilitate the seamless initiation and delivery of services. Through regularly scheduled home visits, the RN evaluates patient and caregiver well-being, monitors the quality and effectiveness of care, and identifies opportunities to enhance health outcomes, safety, and regulatory compliance. The Care Manager, RN proactively communicates clinical concerns, changes in condition, and potential barriers to care with the appropriate team members to ensure timely intervention, effective care coordination, and optimal patient outcomes.</span><span data-ccp-props="{"201341983":0,"335559739":60,"335559740":276}"> </span></p>

<li><span data-contrast="auto">Conducts initial in-person clinical assessments and coordinates service activation, ensuring that all required forms and standard procedures are followed.</span><span data-ccp-props="{"201341983":0,"335559739":60,"335559740":276}"> </span></li>
<li><span data-contrast="auto">Develops, implements, manages patient's plan of care.</span><span data-ccp-props="{"201341983":0,"335559739":60,"335559740":276}"> </span></li>
<li><span data-contrast="auto">making revisions and documenting changes as needed.</span><span data-ccp-props="{"201341983":0,"335559739":60,"335559740":276}"> </span></li>
<li><span data-contrast="auto">Actively coordinates the care of the client through communication and written documentation to the physician, and other disciplines involved in the client’s care and community resources.</span><span data-ccp-props="{"201341983":0,"335559739":60,"335559740":276}"> </span></li>
<li><span data-contrast="auto">Provide ongoing coaching to drive effective utilization of established best practices, encourage caregiver education, and connect families to appropriate resources.</span><span data-ccp-props="{"201341983":0,"335559739":60,"335559740":276}"> </span></li>
<li><span data-contrast="auto">Reviews the caregiver notes daily and appropriately addresses changes in the consumer’s condition or environment.</span><span data-ccp-props="{"201341983":0,"335559739":60,"335559740":276}"> </span></li>
<li><span data-contrast="auto">Ensure continuous compliance with all regulatory standards as well as company policies and procedures.</span><span data-ccp-props="{"201341983":0,"335559739":60,"335559740":276}"> </span></li>

This provides our employees with the opportunity to professionally grow and develop within a role. Compensation decisions are dependent upon a variety of factors which may include, but are not limited to: skill set, years of relevant experience, education, location, and licensure/certifications.</span></p>

The base pay range for this role is between $80,000 and $90,000 per year at the commencement of your first year of employment.
