The goal of this service is to provide necessary care while fostering and emphasizing the patient’s independence in a home environment that will provide them with a range of care options as their needs evolve. The service is designed to provide options for alternative long-term care to persons who meet nursing facility-level care needs, while preserving the dignity, self-respect, and privacy of the patient.&nbsp;&nbsp; </span></p> <p><span data-contrast="none">Working closely with waiver members, caregivers, and other stakeholders in the business, the Health Coach’s goal will be to provide a seamless Customer Service experience, while ensuring that our clients are receiving the care they need.</span></p> <p><strong><span data-contrast="none"><span data-ccp-parastyle="Default">Every Day You Will:</span></span></strong><span data-ccp-props="{&quot;201341983&quot;:0,&quot;335559738&quot;:0,&quot;335559739&quot;:240,&quot;335559740&quot;:240}">&nbsp;</span></p> <ul> <li><span data-contrast="none">Schedule and perform initial home assessments <strong>(virtual and in-person)&nbsp;</strong></span></li> <li><span data-contrast="none">&nbsp;Serve as a field expert to support waiver member and caregiver onboarding experience (as needed) for orientation and remote onboarding to ensure consent and acceptance</span></li> <li><span data-contrast="none">&nbsp;Perform required “home inspections” to ensure safety and compliance of participants’ home and surroundings</span></li> <li><span data-contrast="none">&nbsp;Partner with Care team to monitor Daily Notes submitted by the caregiver for indications that the caregiver may need additional training or support, and document all contact with the caregiver in Salesforce</span></li> <li><span data-contrast="none">&nbsp;Maintain contact with the caregiver, virtual or in person, based on the needs of the waiver member or caregiver identified during the initial assessment</span></li> <li><span data-contrast="none">&nbsp;On a semi-annual basis (or as required by contract) at a minimum, visit the waiver member and caregiver with the waiver Case Manager to establish communication between all members of the care team; perform additional home visits if the need is identified during the monthly contact or through Daily Notes review</span></li> <li><span data-contrast="none">&nbsp;Partner with Care team in successfully transition newly onboarded waiver members to the ongoing program phase</span></li> <li><span data-contrast="none">&nbsp;Communicate any concerns regarding the waiver member or caregivers’ health, surroundings, safety, or unmet needs to staff RN; participate in coordination with the Waiver or assigned Case Manager</span></li> <li><span data-contrast="none">&nbsp;Track all activities through entries into CRM (Salesforce)</span></li> <li><span data-contrast="none">&nbsp;Function within the timeframes of a fast-paced startup in the healthcare environment</span></li> <li><span data-contrast="none">&nbsp;Performs other duties as required or assigned</span></li> </ul> <p><strong>Ideal Candidate Will Possess:</strong></p> <ul> <li><span data-contrast="auto">1+ years of experience working with frail or elderly disabled adults and caregivers</span></li> <li><span data-contrast="auto">1-3 years of medical field experience and/or case management, home health; experience updating and maintaining case notes and completing case reviews</span></li> <li><span data-contrast="auto">The ability to exercise judgement independently and voice noticeable issues or trends</span></li> <li><span data-contrast="auto">Strong verbal and written communication skills; ability to establish strong rapport to effectively communicate, guide, educate, and support waiver members and caregivers</span></li> <li><span data-contrast="auto">&nbsp;Strong problem-solving skills; able to identify issues and escalate when appropriate</span></li> <li><span data-contrast="auto">&nbsp;Excellent time management, critical thinking and active listening skills; able to multitask</span></li> <li><span data-contrast="auto">&nbsp;Possesses a high level of empathy and emotional intelligence</span></li> <li><span data-contrast="auto">&nbsp;Strong proficiency in computer software navigation; Salesforce experience, as plus</span></li> <li><span data-contrast="auto">&nbsp;Comfort in a dynamic and always evolving startup environment</span></li> <li><span data-contrast="auto"> Always maintain patient confidentiality; understands and adheres to HIPAA guidelines</span></li> </ul> <p><strong>Preferred Qualifications:<br></strong></p> <ul> <li><span data-contrast="auto">Bachelor's degree or equivalent work experience is a plus</span></li> <li><span data-contrast="auto">1 year of experience as an EMT (Emergency Medical Technician)&nbsp;</span></li> <li><span data-contrast="auto">3+ years of experience working with frail or elderly disabled adults and caregivers</span></li> </ul> <p><strong><br>Why work at FreedomCare?</strong></p> <p><span style="font-weight: 400;">We are here for you and committed to providing a best-in-class employee experience. This provides our employees with the opportunity to professionally grow and develop within a role. The base pay range for this role is between $23.00 and $25.00 an hour at the commencement of your first year of employment. Compensation decisions are dependent upon a variety of factors which may include, but are not limited to: skill set, years of relevant experience, education, location, and licensure/certifications.</span></p></div><div class="title">Compensation Range</div><div class="pay-range"><span>$23</span><span class="divider">&mdash;</span><span>$25 USD</span></div></div></div>
