This includes coordination for utilization review, legal matters, and employment needs for clients.</li> <li>Provide individual case coordination sessions and services, including transportation coordination.</li> <li>Assist in developing aftercare plans, offering referrals and contact information for aftercare providers, and addressing any needs from outside providers.</li> <li>Support clinical services by assisting with group sessions and engaging clients weekly to enhance their participation in the program.</li> <li>Document all interactions with clients, families, referrals, and outside providers in Carelogic, adhering to program timelines.</li> <li>Collaborate with the client navigation team to address aftercare needs effectively.</li> <li>Provide onsite support for Enhanced Outpatient Sessions (EOS) appointments between clients and the Client Navigation team.</li> <li>Perform other duties as assigned.</li> </ul> <h3>Education, Licensure, &amp; Experience:</h3> <ul> <li>High School Diploma or equivalent is required.</li> <li>1 year of experience in a drug and alcohol or mental health setting is preferred.</li> <li>A valid Driver’s License is preferred.</li> <li>CPR/BLS certification is preferred.</li> </ul> <p>We look forward to welcoming a dedicated Case Coordinator to our team at Pyramid Healthcare at Silvermist.
