Join us as we build a new program dedicated to recovery and make an immediate impact on the lives of clients and their families.</p> <h2 data-section-id="115i9ww" data-start="913" data-end="932">Position Summary</h2> <p data-start="933" data-end="1314">We are seeking a <strong data-start="950" data-end="990">dedicated and organized Case Manager</strong> to support clients and families throughout the continuum of care. This role is essential to client success and includes coordinating services, communicating with referral sources and external providers, assisting with admissions processes, and supporting aftercare planning to ensure smooth transitions following treatment.</p> <h2 data-section-id="18w7bso" data-start="1316" data-end="1339">Key Responsibilities</h2> <ul data-start="1340" data-end="2613"> <li data-section-id="tzpaps" data-start="1340" data-end="1436">Coordinate client care activities, including appointments, schedules, and treatment planning</li> <li data-section-id="1skkag1" data-start="1437" data-end="1525">Communicate with referral sources and outside providers to ensure continuity of care</li> <li data-section-id="e5ke2v" data-start="1526" data-end="1669">Support the admissions process by verifying documentation, scanning records, and initiating communication with external providers as needed</li> <li data-section-id="1cds1td" data-start="1670" data-end="1764">Conduct orientations with clients and families in collaboration with the primary therapist</li> <li data-section-id="c5yt13" data-start="1765" data-end="1915">Provide case management services including transportation coordination, case management sessions, and support related to legal or employment needs</li> <li data-section-id="3bam56" data-start="1916" data-end="1999">Facilitate educational or skill-building groups to support clinical programming</li> <li data-section-id="ilom4i" data-start="2000" data-end="2126">Develop and document comprehensive aftercare plans in collaboration with the treatment team and business development staff</li> <li data-section-id="1mdbyy4" data-start="2127" data-end="2230">Provide aftercare documentation to appropriate external providers and ensure follow-up coordination</li> <li data-section-id="1ic2dmn" data-start="2231" data-end="2330">Maintain accurate and timely documentation in the electronic clinical record system (CareLogic)</li> <li data-section-id="16xizvv" data-start="2331" data-end="2440">Participate in treatment team meetings and maintain collaborative communication with counselors and staff</li> <li data-section-id="101lqbc" data-start="2441" data-end="2539">Ensure compliance with confidentiality standards, licensing requirements, and program policies</li> <li data-section-id="19hb9ps" data-start="2540" data-end="2613">Complete required trainings and perform additional duties as assigned</li> </ul> <h2 data-section-id="1fm6vrb" data-start="2615" data-end="2632">Qualifications</h2> <p data-start="2634" data-end="2647"><strong data-start="2634" data-end="2647">Education</strong></p> <p></p> <ul> <li data-ogsc="black"> <div data-ogsc="" data-olk-copy-source="MessageBody">High School Diploma or GED&nbsp;required;
