We know that the journey to recovery is unique for every individual, and our patient-centered approach ensures we meet patients where they are, providing evidence-based therapies with unwavering support in a compassionate environment that fosters healing and growth.   <h3>About this Role   </h3> The Case Manager is a key member of the interdisciplinary treatment team, responsible for coordinating and advocating for comprehensive services that address the biopsychosocial needs of clients in outpatient substance use and co-occurring mental health treatment. Utilizing a trauma-informed, person-centered approach, the Case Manager supports client recovery through care coordination, resource navigation, and ongoing support to address barriers to treatment engagement and retention.  <h3>Some of the Benefits  </h3> <ul> <li>Healthcare Coverage: Medical, dental and vision insurance </li> <li>Time Off: Flexible time off with generous PTO, floating paid holidays, and paid volunteer days  </li> <li>Re-Fuel Days: 4 additional paid days off per year for mental health, rest, or continuing education (CEU) activities. </li> <li>Employee Assistance Program (EAP): Free, confidential access to therapist sessions, legal guidance, financial resources, health coaching, and more - to support your overall well-being. </li> <li>401K Match up to 5% </li> <li>100% employer-paid short-term and long-term disability and employer-sponsored life insurance  </li> <li>SoFi student loan interest rate discount, Rightway Health Concierge, 1 year free pet Telehealth through Pawp, and Perkspot Discount Marketplace</li> <li>A long-term commitment to reasonable caseloads and representation on the leadership team  </li> <li>An opportunity to get in at the ground level and shape how we grow! </li> </ul> Key Responsibilities (include but are not limited to):  <ul> <li>Conduct comprehensive case management assessments upon admission and at regular intervals.  </li> <li>Develop and implement individualized care plans in collaboration with the client and treatment team.  </li> <li>Coordinate access to social, financial, medical, legal, housing, and vocational resources to address social determinants of health.  </li> <li>Collaborates with Peer Support Specialists to work together in assisting patients in addressing their social determinants of health   </li> <li>Monitor progress toward care plan goals and update plans as needed.  </li> <li>Maintain ongoing communication with internal clinical team members and external service providers to ensure continuity of care.  </li> <li>Facilitate warm hand-offs and follow-ups to support treatment retention.  </li> <li>Provide psychoeducation and support on navigating systems such as Medicaid, legal aid, child welfare, etc.  </li> <li>Advocate for clients' needs while promoting autonomy and empowerment.  </li> <li>Maintain accurate, timely documentation in accordance with Medicaid, Medicare, and CARF standards (including contact logs, service plans, progress notes).  </li> <li>Participate in interdisciplinary team meetings, treatment planning, and staff development trainings.  </li> <li>Identify and address potential risks to client safety and report concerns according to protocol.  </li> <li>Safely transport patients to and from appointments, facilities, and designated locations using a personal vehicle while maintaining professionalism and patient confidentiality. </li> </ul> Qualifications    <ul> <li>Case Management Certification or equivalent experience preferred.  </li> <li>At least one year of experience working with individuals with SUD and/or co-occurring disorders required.  </li> <li>Knowledge of trauma-informed care, harm reduction, and motivational interviewing principles.  </li> <li>Strong interpersonal, organizational, and advocacy skills.  </li> <li>Familiarity with state specific Medicaid documentation standards and local community resources.  </li> <li>Proficiency in EMR systems (Athena EMR experience preferred).  </li> <li>Must possess a valid driver’s license in good standing. </li> <li>Must maintain an active personal automobile insurance policy that meets or exceeds state minimum coverage requirements and provide proof of insurance upon hire and as requested thereafter. </li> </ul> <hr> Schedule: This role follows the anticipated schedule of Monday - Friday, 8:30a - 5:00p EST.
