Case Manager- Community Based (Adult/Youth)

Working with a supportive management staff and a company that puts their clients and their employees first, Crossroads Roads Health is an organization built on mutual respect and a work-life balance that is in need of a passionate, empathic individual to perform the following tasks:</p> <ul> <li>Provides therapeutic interventions in the home or community and linking clients to resources</li> <li>Assesses clients’ development, psychological and psychiatric needs in order to make referrals.</li> <li>Advocates on behalf of clients.</li> <li>Monitors clinical symptoms.</li> <li>Functions as a gatekeeper for other resources.</li> <li>Performs crisis intervention, as needed.</li> <li>Meet monthly productivity metrics</li> </ul> <p>Position Requirements:</p> <ul> <li>High School Diploma or equivalent, Associate’s Degree preferred, with 3 years of related experience</li> <li>QMHS/QBHS within 15 days of hire (employer provided)</li> <li>Demonstrated passion to advocate on behalf of others</li> <li>Demonstrated self-starter and independent thinker who can work in the field as well as in the office</li> <li>Organized with good time management skills necessary</li> <li>Must have an acceptable driving record 4 (four) points or less, reliable transportation and carry automobile insurance with minimum liability of $100,000/$300,000, and property damage of $50,000.</li> </ul> <p>The anticipated hiring range for this position is $20.00 - $23.00 per hour.
