<p style="margin: 0px;"><span style="font-family: verdana, geneva; font-size: 12pt;">The <strong>Case Manager III</strong> facilitates admission intakes, treatment planning, discharge planning, obtaining collateral, clinical interventions, crisis management, assists in the legal process and initiates personalized care for individuals who are in crisis and stabilizing from crisis. Works collaboratively with the interdisciplinary treatment team to coordinate care and discharge planning for Individuals, their family members, clinics, local providers, and/or other referral sources. This position will carry a caseload.</span></p>
<ul>
<li><span style="font-family: verdana, geneva; font-size: 12pt;">Conducts a thorough, accurate, clinically sound Psychosocial Assessment with the individual in a timely manner.</span></li>
<li style="line-height: 115%; margin: 0in 0in 0in 0px; font-size: 11pt; font-family: Calibri, sans-serif;"><span style="font-size: 12pt; line-height: 115%; font-family: verdana, geneva;">Ability to review and sign off on Psychosocial Assessments completed by Certified AAC or associate level staff members, if fully licensed.</span></li>
<li style="line-height: 115%; margin: 0in 0in 0in 0px; font-size: 11pt; font-family: Calibri, sans-serif;"><span style="font-size: 12pt; line-height: 115%; font-family: verdana, geneva;">Educates and informs individuals of consents for treatment and authorizations for use and disclosures.</span></li>
<li style="line-height: 115%; margin: 0in 0in 0in 0px; font-size: 11pt; font-family: Calibri, sans-serif;"><span style="font-size: 12pt; line-height: 115%; font-family: verdana, geneva;">Participates regularly in Interdisciplinary Treatment Team meetings.</span></li>
<li style="line-height: 115%; margin: 0in 0in 0in 0px; font-size: 11pt; font-family: Calibri, sans-serif;"><span style="font-size: 12pt; line-height: 115%; font-family: verdana, geneva;">Collaborates with the Treatment Team to develop and implement an individualized person-centered Treatment Plan that is appropriate for brief intervention. Complete the signed Treatment Plan and review with each individual and any other involved parties.</span></li>
<li style="line-height: 115%; margin: 0in 0in 0in 0px; font-size: 11pt; font-family: Calibri, sans-serif;"><span style="font-size: 12pt; line-height: 115%; font-family: verdana, geneva;">Gather collateral from the individual and involved parties to provide a thorough assessment of an individuals' needs for treatment and discharge.</span></li>
<li style="line-height: 115%; margin: 0in 0in 0in 0px; font-size: 11pt; font-family: Calibri, sans-serif;"><span style="font-family: verdana, geneva; font-size: 12pt;"><span style="line-height: 115%;">Maintain a working knowledge of the Involuntary Treatment Act (ITA) and Evaluation and Treatment (E&T) Involuntary Commitment process. </span><span style="line-height: 115%;">Monitor the petition process to ensure timeliness are adhered to, and forms are complete and thorough. When subpoenaed, attend E&T hearings.</span></span></li>
<li style="line-height: 115%; margin: 0in 0in 0in 0px; font-size: 11pt; font-family: Calibri, sans-serif;"><span style="font-size: 12pt; line-height: 115%; font-family: verdana, geneva;">Actively coordinate care and provide referrals for individuals to ensure services are coordinated with both facility staff and external providers.</span></li>
<li style="line-height: 115%; margin: 0in 0in 0in 0px; font-size: 11pt; font-family: Calibri, sans-serif;"><span style="font-size: 12pt; line-height: 115%; font-family: verdana, geneva;">Identify and address gaps in service needs for participating parties in service delivery to the individual and make appropriate recommendations to meet the individuals’ needs upon discharge.</span></li>
<li style="line-height: 115%; margin: 0in 0in 0in 0px; font-size: 11pt; font-family: Calibri, sans-serif;"><span style="font-size: 12pt; line-height: 115%; font-family: verdana, geneva;">Collaborate with Interdisciplinary Treatment Team to engage, monitor and communicate with the individual ensuring that safety is maintained while meeting the individuals' needs.</span></li>
<li style="line-height: 115%; margin: 0in 0in 0in 0px; font-size: 11pt; font-family: Calibri, sans-serif;"><span style="font-size: 12pt; line-height: 115%; font-family: verdana, geneva;">Participates in redirecting individual behavior while maintaining a safe milieu when necessary. Documents all services and individual activities appropriately.</span></li>
<li style="line-height: 115%; margin: 0in 0in 0in 0px; font-size: 11pt; font-family: Calibri, sans-serif;"><span style="font-size: 12pt; line-height: 115%; font-family: verdana, geneva;">Provides internal education regarding DBT, trauma informed care, harm reduction, and service plan documentation to enhance staffs' ability to effectively address and manage crisis situations.</span></li>
<li style="line-height: 115%; margin: 0in 0in 0in 0px; font-size: 11pt; font-family: Calibri, sans-serif;"><span style="font-size: 12pt; line-height: 115%; font-family: verdana, geneva;">Provides case management services to promote recovery and community reintegration.</span></li>
<li style="line-height: 115%; margin: 0in 0in 0in 0px; font-size: 11pt; font-family: Calibri, sans-serif;"><span style="font-size: 12pt; line-height: 115%; font-family: verdana, geneva;">Maintain the clinic record, including documentation of activities performed as part of the service delivery process. Obtain signatures from individuals regarding treatment, as necessary. Document all services and individuals’ activities in the medical record regarding brief interventions, coordination of care, discharge planning, treatment planning, and any other pertinent information.</span></li>
<li style="margin: 0in 0in 0in 0px; font-size: 11pt; font-family: Calibri, sans-serif;"><span style="font-size: 12pt; font-family: verdana, geneva;">Prepares clinical reports and individualized service plans utilizing agency electronic clinical documentation software. Ensures documentation complies with best clinical practices, contracts, regulatory, and other legal requirements.</span></li>
<li style="line-height: 115%; margin: 0in 0in 0in 0px; font-size: 11pt; font-family: Calibri, sans-serif;"><span style="font-size: 12pt; line-height: 115%; font-family: verdana, geneva;">Maintain a working knowledge of the Washington State behavioral health system and potential funding sources.</span></li>

<li><span style="font-size: 12pt; font-family: verdana, geneva;">Master’s degree in behavioral health, Social Work, Counseling, Psychology, or related discipline </span></li>
<li><span style="font-family: verdana, geneva; font-size: 12pt;"><span class="ui-provider a b c d e f g h i j k l m n o p q r s t u v w x y z ab ac ae af ag ah ai aj ak">Unrestricted license as a Licensed Mental Health Professional (LMHP) in Washington State and/or Licensed Independent Clinical Social Worker (LICSW), Licensed Mental Health Counselor (LMHC), Licensed Marriage and Family Therapist (LMFT), <strong><em>or</em> </strong>associate license (LICSWA, LMHCA, etc.)</span></span></li>
<li><span style="font-size: 12pt; font-family: verdana, geneva;">3 years of experience in mental health, substance abuse treatment, or relevant work</span></li>
<li><span style="font-family: verdana, geneva; font-size: 12pt;">Knowledge of the Washington State behavioral health system of care and resources within it</span></li>
<li><span style="font-family: verdana, geneva; font-size: 12pt;"><strong>The Company has a mandatory vaccination policy.
