<p style="margin: .2pt 7.75pt .0001pt 5.0pt;"><span style="font-family: verdana, geneva; font-size: 12pt;">The <strong>Case Manager</strong> is responsible for managing the care of individuals, including both youth (adolescents) and adults, who are in crisis and stabilizing from acute episodes. This role involves facilitating admission intakes, treatment planning, discharge planning, obtaining collateral, assisting in the legal process, and initiating personalized care plans. The Case Manager works closely with an interdisciplinary treatment team to coordinate care and discharge planning for both youth and adult individuals, their families, schools (for youth), local providers, and other referral sources. This position also carries a caseload and serves as an ambassador to community partners.</span></p>
<ul>
<li><span style="font-family: verdana, geneva; font-size: 12pt;"><strong>Admission & Consent</strong></span>
<ul>
<li><span style="font-family: verdana, geneva; font-size: 12pt;">Facilitate admission intakes, educating and informing individuals and their families about consents for treatment and authorizations for use and disclosures.</span></li>
</ul>
</li>
<li><span style="font-family: verdana, geneva; font-size: 12pt;"><strong>Treatment Planning:</strong></span>
<ul>
<li><span style="font-family: verdana, geneva; font-size: 12pt;">Collaborate with the Interdisciplinary Treatment Team to develop and implement individualized, person-centered Treatment Plans appropriate for brief intervention.</span></li>
<li><span style="font-family: verdana, geneva; font-size: 12pt;">Complete and review signed Treatment Plans with each individual and their families (where applicable) to ensure understanding and agreement.</span></li>
</ul>
</li>
<li><span style="font-family: verdana, geneva; font-size: 12pt;"><strong>Collateral Gathering & Assessment:</strong></span>
<ul>
<li><span style="font-family: verdana, geneva; font-size: 12pt;">Gather collateral information from individuals, family members, schools (for youth), and other involved parties to conduct a thorough assessment of needs for treatment and discharge planning.</span></li>
</ul>
</li>
<li><span style="font-family: verdana, geneva; font-size: 12pt;"><strong>Care Coordination:</strong></span>
<ul>
<li><span style="font-family: verdana, geneva; font-size: 12pt;">Actively coordinate care and provide referrals to ensure services are aligned with both facility staff and external providers.</span></li>
<li><span style="font-family: verdana, geneva; font-size: 12pt;">Identify and address gaps in service needs, making appropriate recommendations to meet the individual’s needs upon discharge.</span></li>
</ul>
</li>
<li><span style="font-family: verdana, geneva; font-size: 12pt;"><strong>Discharge Planning:</strong></span>
<ul>
<li><span style="font-family: verdana, geneva; font-size: 12pt;">Collaborate with the interdisciplinary treatment team to engage with, monitor, and communicate with individuals and their families, ensuring safety and meeting their needs during the discharge process.</span></li>
</ul>
</li>
<li><span style="font-family: verdana, geneva; font-size: 12pt;"><strong>Documentation:</strong></span>
<ul>
<li><span style="font-family: verdana, geneva; font-size: 12pt;">Maintain accurate clinic records, documenting all activities related to service delivery, including brief interventions, coordination of care, discharge planning, and treatment planning.</span></li>
<li><span style="font-family: verdana, geneva; font-size: 12pt;">Obtain necessary signatures from individuals or their guardians regarding treatment and other required documents.</span></li>
</ul>
</li>
<li><span style="font-family: verdana, geneva; font-size: 12pt;"><strong>Community & System Knowledge:</strong></span>
<ul>
<li><span style="font-family: verdana, geneva; font-size: 12pt;">Maintain a working knowledge of the Pennsylvania State behavioral health system, including potential funding sources, 302 involuntary legal processes, and resources specific to both youth and adults.</span></li>
</ul>
</li>
<li><span style="font-family: verdana, geneva; font-size: 12pt;"><strong>Professional Development:</strong></span>
<ul>
<li><span style="font-family: verdana, geneva; font-size: 12pt;">Participate in ongoing education, including in-services, training, and other activities to maintain and improve competency.</span></li>
<li><span style="font-family: verdana, geneva; font-size: 12pt;">Demonstrate competency through post-testing, skill observation, and performance assessments as conducted by direct supervisors.</span></li>
</ul>
</li>
<li><span style="font-family: verdana, geneva; font-size: 12pt;">Performs all other duties as assigned</span></li>

<p style="margin: 0px;"><span style="font-family: verdana, geneva; font-size: 12pt;">Bachelor’s degree in behavioral health or related field <strong><u>OR </u></strong>4 years of behavioral health experience</span></p>
</li>
<li><span style="font-size: 12pt; font-family: verdana, geneva;">Knowledge of the Pennsylvania State behavioral health system of care and resources within it</span></li>
<li>
