This role involves facilitating admission intakes, treatment planning, discharge planning, obtaining collateral, assisting in the legal process, and initiating personalized care plans. The Case Manager works closely with an Interdisciplinary Treatment Team to coordinate care and discharge planning for individuals, their families, local providers, and other referral sources. This position also carries a caseload and serves as an ambassador to community partners.</span></p> <ul style="text-align: left;"> <li><span style="font-size: 12pt; font-family: verdana, geneva;">Educates and informs individuals of consents for treatment and authorizations for use and disclosures.</span></li> <li><span style="font-size: 12pt; font-family: verdana, geneva;">Participates regularly in Interdisciplinary treatment team meetings.</span></li> <li><span style="font-size: 12pt; font-family: verdana, geneva;">Collaborates with the Treatment Team to develop and implement an individualized person-centered Treatment Plan that is appropriate for brief intervention. Complete the signed Treatment Plan and review with each individual and any other involved parties.</span></li> <li><span style="font-size: 12pt; font-family: verdana, geneva;">Gather collateral from the individual and involved parties to provide a thorough assessment of what an individuals' needs are for treatment and discharge.</span></li> <li><span style="font-size: 12pt; font-family: verdana, geneva;">Actively&nbsp;coordinate&nbsp;care and provide referrals&nbsp;for&nbsp;individuals to&nbsp;ensure&nbsp;services&nbsp;are&nbsp;coordinated&nbsp;with&nbsp;both facility&nbsp;staff and external providers.</span></li> <li><span style="font-size: 12pt; font-family: verdana, geneva;">Identify and address gaps in service needs for participating parties in service delivery to the individual; and make appropriate recommendations to meet individuals needs upon discharge.</span></li> <li><span style="font-size: 12pt; font-family: verdana, geneva;">Collaborate with interdisciplinary treatment team to engage, monitor and communicate with the individual ensuring that safety is maintained while meeting the individuals' needs.</span></li> <li><span style="font-size: 12pt; font-family: verdana, geneva;">Maintain the clinic record, including documentation of activities performed as part of the service delivery process. Obtain signatures from individuals regarding treatment, as necessary. Document all services and individuals' activities in the medical record regarding brief interventions, coordination of care, discharge planning, treatment planning, and any other pertinent information.</span></li> <li><span style="font-size: 12pt; font-family: verdana, geneva;">Maintain a working knowledge of the Virginia Involuntary Commitment process.</span></li> <li><span style="font-size: 12pt; font-family: verdana, geneva;">Maintain&nbsp;a&nbsp;working&nbsp;knowledge&nbsp;of&nbsp;the&nbsp;DBHDS and DMAS behavioral health system</span></li> <li><span style="font-size: 12pt; font-family: verdana, geneva;">Participate in individual and group supervision as required. Participate in ongoing education, including in-services, training, and other activities to maintain and improve competency.
