Responsibilities include:  <ul> <li>Provide mobile and field-based case management services in the areas of independent living skills, housing stabilization, money management, community integration, employment linkage, benefits establishment, linkage to VA partners and community providers for substance abuse, primary and mental healthcare, housing navigation, and all other services necessary to assist clients in reaching their goals.</li> <li>Provide strengths-based case management and service coordination services designed to assist veterans in obtaining and maintaining stable employment and housing.</li> <li>Collaborate with clients and other VASH and VA staff to develop a Housing Stability Plan (HSP) for each client that addresses their short- and long-term goals.</li> <li>Provide on-going case management support to assess HSP progress and ensure treatment plan outcomes are met or adjusted as necessary.</li> <li>Provide supportive services focused on enhancing clients’ ability to independently problem solve, utilize effective coping skills and manage and coordinate own care.</li> <li>Utilize evidence-based practices in service delivery such as intensive case management, Motivational Interviewing, Harm Reduction, Critical Time Intervention and Housing First practices.</li> </ul>  WHAT YOU BRING We’re looking for candidates that: <ul> <li>Demonstrate knowledge of and experience with advanced and evidence-based interventions such as, but not limited to: Critical Time Intervention, Harm Reduction, Crisis Intervention, Motivational Interviewing</li> <li>Proficiency with computers and technologies used for work outputs including: Microsoft Office Software; Database Systems</li> <li>A non-judgmental and respectful approach when working with individuals who present for services with urgent and multiple case management and health needs. </li> <li>Professionalism and mature judgement. </li> <li>High level of motivation and self-starting demeanor.</li> <li>The flexibility, adaptability, and capability to work effectively in a fast-paced, professional environment with dynamic priorities. </li> </ul> PREFERRED QUALIFICATIONS <ul> <li> Current and active licensure or registration with the California Board of Behavioral Sciences (e.g., ACSW, LCSW).</li> </ul> MINIMUM QUALIFICATIONS <ul> <li>Master's Degree in Social Service or Clinical field (MSW, MFT, Clinical Counselor, Psychology etc.)</li> <li>One (1) years of experience in healthcare, case management, homeless services or working with vulnerable populations.</li> </ul> MANDATORY REQUIREMENTS For this role, candidates must: <ul> <li>Be able and willing to work flexible hours which may include evenings or weekends</li> <li>Have employment eligibility verification</li> <li>Have or be able and willing to obtain CPR/First Aid training</li> <li>Provide proof of full COVID-19 vaccination</li> <li>Successfully complete the following as a condition of hire: <ul> <li>Tuberculosis Test</li> <li>Background Screening</li> <li>Drug Test</li> </ul> </li> </ul> DRIVING Driving is an essential function of this position.
