Here, you are not just joining a workplace, you are becoming part of a mission that truly matters.</div>Summary: The Community Resource Navigator will serve as a primary point of contact for healthcare patients, as well as the communities we serve, in an ambulatory care setting.  They will support individuals and families entering the healthcare system and assist with utilization and retention of benefits and community resources. They will also focus on helping eliminate barriers to all JPCHC services.  This position will float to clinics as needed.    Essential Duties and Responsibilities: Essential Duties include, but are
