<p>Eliot's Developmental Disabilities division is seeking a <strong>Community Supports Case Manager</strong>. This division offers support and opportunities for individuals to enrich their lives, obtain housing and employment, develop personal skills and competencies, and enjoy a full range of community activities while developing a network of caring relationships. </p>
<p>The ideal candidate will have experience working with adults with developmental disabilities and will have the ability to advocate for community supports and services on behalf of the individuals.</p>

<p><strong>Responsibilities:</strong></p>
<ul>
<li>Assist in completing assessments and Individual Service Plans(ISP)/Plans of Care(POC) for assigned cases in a timely manner and as indicated by DDS and MRC policies and standards.</li>
<li>Ensure individuals are involved in identifying and developing their own goals.</li>
<li>Document daily progress notes for all individuals in accordance with Eliot Policy and DDS/MRC documentation and billing requirements.</li>
<li>Develop and maintain a positive and professional working relationship with individuals, families and providers. This includes maintaining a set schedule with individuals, timely communication and follow up on issues and requests and being on time for appointments, meetings and activities.</li>
<li>Coordinate service delivery for cases assigned i.e., case conferences, Section 8 Housing, SSI and advocate for the consumer with outside agencies and providers.</li>
<li>Provide service linkages to other providers as necessary. Identify and schedule appointments, for/with individuals, as needed or required.</li>

<p><strong>Qualifications:</strong></p>
<ul>
<li>Bachelor’s degree or equivalent experience</li>
<li>Experience working with intellectually disabled or brain injured adults.</li>
<li>Ability to manage a client caseload in an outreach setting.</li>
<li>Valid driver’s license and a reliable vehicle.</li>
<li>Ability to work a community based position that services the Lawrence, Lowell, Burlington and Arlington areas. Main office located in Lexington MA. </li>
