<p><strong>Job Summary</strong></p>
<p>The Company is currently hiring a <strong>Dialysis NP/PA</strong> to provide on-site care to long term care residents and outpatient residents in a dialysis center. This is a <strong>full-time</strong> position with a <strong>mix of Dialysis Care and Primary Care</strong> - ideal for a provider who values meaningful patient relationships and a collaborative care environment.</p>
<p><strong>Responsibilities</strong></p>
<ul>
<li>Provide primary care services to dialysis patients in skilled nursing and long-term care facilities as well as outpatient dialysis patients</li>
<li>Perform physical exams and medical history assessments</li>
<li>Diagnose and treat chronic and acute health conditions</li>
<li>Order and interpret diagnostic tests and lab results</li>
<li>Develop and adjust individualized care plans</li>
<li>Prescribe medications and monitor treatment effectiveness </li>
<li>Collaborate with facility staff and interdisciplinary teams</li>
<li>Collaborate with dialysis center medical director for improvement in patient care and quality metrics</li>
<li>Interact with skilled nursing and long term care physicians and non-physician practitioner to assure good coordination of care and reduction of avoidable hospitalizations</li>
<li>Ensure a safe, respectful, and supportive environment for patients</li>
<li><strong>Clinical management</strong>: Perform comprehensive assessments, diagnose conditions, and adjust dialysis treatment plans and medications in collaboration with a nephrologist.</li>
<li><strong>Patient monitoring:</strong> Continually oversee patients' health during and after dialysis, interpret lab results, and respond to any complications.</li>
<li><strong>Interdisciplinary coordination:</strong> Lead and collaborate with the facility's nursing staff, dialysis technicians, and other healthcare providers to ensure cohesive patient care.</li>
<li><strong>Patient and family education:</strong> Educate patients and their families on kidney disease, the dialysis process, and critical lifestyle modifications like diet and fluid intake.</li>
<li><strong>Administrative documentation: </strong>Maintain precise and timely patient records, manage medication orders, and participate in quality improvement initiatives.</li>
<li><strong>Patient advocacy:</strong> Serve as the primary point of contact for patients and families, providing clinical and emotional support while coordinating care with specialists.</li>
</ul>
<p><strong>Requirements</strong></p>
<ul>
<li>Active and unrestricted <strong>Nurse Practitioner, Physician Assistant license</strong></li>
<li>State-specific license in good standing in Louisiana </li>
<li>Minimum <strong>2 years of experience as a dialysis NP/PA</strong></li>
<li>Willingness to work with dialysis patients in skilled nursing and long term care facilities</li>
<li>Strong communication, clinical, and critical thinking skills</li>
<li>Passion for elder care and person-centered healthcare</li>
<li>Must be able to lift a minimum of 20 pounds and stand / walk / work on your feet up to 6-8 hours per day</li>
