<p><strong>Job Title: In-Home Dialysis Care Partner<br></strong><strong>Location:</strong> Thousand Oaks, CA (91359)<br><strong>Compensation:</strong>&nbsp;$25 per hour</p> <p><strong>About the Role:</strong><br>We are looking for compassionate and reliable&nbsp;<strong>Care Partners with at least 1 year of Dialysis machine experience</strong>&nbsp;to provide quality support for patients receiving dialysis in the comfort of their own homes. As a Care Partner, you will work closely with patients undergoing in-home dialysis, ensuring their environment is safe, they are prepped and supported for treatment, and that their overall wellbeing is monitored.</p> <p>You’ll be matched with patients based on both&nbsp;<strong>skill fit</strong>&nbsp;and&nbsp;<strong>personality</strong>, and supported by a clinical team<strong>.</strong></p> <p><strong>Key Responsibilities:</strong></p> <ul> <li>Conduct &amp; Support patient setup for dialysis treatments (e.g., NxStage, Tablo or equivalent machines)</li> <li>Provide companionship, emotional reassurance, and schedule adherence</li> <li>Monitor safety and assist with light activities of daily living (ADLs)</li> <li>Maintain communication with clinic nurses and care coordinators</li> <li>Document any concerns or changes observed during support hours</li> </ul> <p><strong>Qualifications:</strong></p> <ul> <li>Prior experience supporting dialysis patients (home or clinical setting)</li> <li>Preferred: Familiarity with home dialysis machines</li> <li>Reliable, empathetic, and detail-oriented</li> <li>Strong interpersonal communication</li> <li>Must be dependable</li> <li>Must pass background check and vetting via Herewith platform</li> </ul> <p><strong>What We Offer:</strong></p> <ul> <li>Consistent weekly hours at $25/hour</li> <li>Stable scheduling tailored to the client's care plan</li> <li>Training support and placement assistance</li> <li>Work independently while being supported by a clinical care team</li> </ul> <p>&nbsp;</p>
