About the role
Key Responsibilities
- Spend the majority of time in the community developing employment opportunities and supporting individuals
- Build and maintain relationships with local employers, Vocational Rehabilitation, and community partners
- Provide individualized, strengths-based employment support using a motivational, stage-matched approach
- Assist individuals in identifying strengths, interests, and goals and incorporate them into Wellness/Treatment Plans
- Support job readiness through skill-building, coaching, and ongoing encouragement
- Help individuals access employment-related and supportive resources (housing, financial, healthcare, etc.)
- Collaborate with the integrated care team to ensure coordinated, whole-person support
- Monitor progress toward employment goals and adjust plans as needed
- Complete timely, accurate documentation and meet service expectations
- Participate in team meetings, supervision, and ongoing professional development
Education, Skills, Knowledge and Expertise
- High School/GED required
- Associate’s/Bachelor’s degree in a human service-related field preferred
- Qualified addiction professional;
- Bachelor’s degree in a human services field from a college or university included in the U.S. Department of Education’s database of accredited schools at http:/ope.ed.gov/accreditation ;
- Any four-year degree or combination of higher education and qualifying experience;
- Four years of qualifying experience; or
- Associate of Applied Science in Behavioral Health Support degree as designated by the department.
- Qualifying experience must include delivery of service to individuals with mental illness, substance use disorders, or developmental disabilities.
- A valid driver's license and agency established minimum automobile coverage required
