Quarterly travel to Tampa, FL. 8:00am-5:00pm Monday-Friday with 2 days determined with supervisor being 10:00am-7:00pm.</em></strong></span></span></p> <p><span style="font-size: 12pt; font-family: verdana, geneva, sans-serif;"><strong>What You'll Do</strong></span></p> <p><span style="font-size: 12pt; font-family: verdana, geneva, sans-serif;">As a Family Resource Specialist, you will provide direct support to patients and families in addressing social needs, working&nbsp;collaboratively with community partners and care team members to close gaps&nbsp;related to non-medical drivers of health. In this role, you will assist in building and&nbsp;strengthening community relationships, identifying and maintaining a robust</span><br><span style="font-size: 12pt; font-family: verdana, geneva, sans-serif;">network of resources, and offering non-clinical support, guidance, and&nbsp;education to families navigating the unique challenges of caring for children&nbsp;with complex medical and behavioral health needs. In this position, you will:</span></p> <ul> <li style="font-size: 12pt; font-family: verdana, geneva, sans-serif;"><span style="font-size: 12pt; font-family: verdana, geneva, sans-serif;">Identify resources and community support available to the family or caregivers</span></li> <li style="font-size: 12pt; font-family: verdana, geneva, sans-serif;"><span style="font-size: 12pt; font-family: verdana, geneva, sans-serif;">Provide information, referrals, and non-clinical assistance with linkage to medical, psychosocial, and basic needs services</span></li> <li style="font-size: 12pt; font-family: verdana, geneva, sans-serif;"><span style="font-size: 12pt; font-family: verdana, geneva, sans-serif;">Empower families to advocate for themselves by providing education, guidance, and supportive tools to navigate systems and access needed services.</span></li> <li style="font-size: 12pt; font-family: verdana, geneva, sans-serif;"><span style="font-size: 12pt; font-family: verdana, geneva, sans-serif;">Assist with accessing specific programs and supports to address psycho-social needs both within Imagine Pediatrics and in the larger community</span></li> <li style="font-size: 12pt; font-family: verdana, geneva, sans-serif;"><span style="font-size: 12pt; font-family: verdana, geneva, sans-serif;">Advocate with community agencies for, and on behalf, of clients and families to decrease service gaps and remove barriers to services</span></li> <li style="font-size: 12pt; font-family: verdana, geneva, sans-serif;"><span style="font-size: 12pt; font-family: verdana, geneva, sans-serif;">Connect with relevant internal staff and providers to assist with system navigation, including eligibility, pediatrics, behavioral health, public health, etc.</span></li> <li style="font-size: 12pt; font-family: verdana, geneva, sans-serif;"><span style="font-size: 12pt; font-family: verdana, geneva, sans-serif;">Assist in development of an internal community resource library (but do not create clinical library materials)</span></li> <li style="font-size: 12pt; font-family: verdana, geneva, sans-serif;"><span style="font-size: 12pt; font-family: verdana, geneva, sans-serif;">Provide linkage to community-based resources and school-based services</span></li> <li style="font-size: 12pt; font-family: verdana, geneva, sans-serif;"><span style="font-size: 12pt; font-family: verdana, geneva, sans-serif;">Network with new and established community resources to increase knowledge of services</span></li> <li style="font-size: 12pt; font-family: verdana, geneva, sans-serif;"><span style="font-size: 12pt; font-family: verdana, geneva, sans-serif;">Administratively document memoranda of understanding (MOU) for partnerships with outside community agencies</span></li> <li style="font-size: 12pt; font-family: verdana, geneva, sans-serif;"><span style="font-size: 12pt; font-family: verdana, geneva, sans-serif;">Develop effective working relationships with interdisciplinary care teams</span></li> <li style="font-size: 12pt; font-family: verdana, geneva, sans-serif;"><span style="font-size: 12pt; font-family: verdana, geneva, sans-serif;">Participate in ongoing scheduled consultations with an interdisciplinary team regarding administrative and non-clinical resources provided to family and caregivers</span></li> <li style="font-size: 12pt; font-family: verdana, geneva, sans-serif;"><span style="font-size: 12pt; font-family: verdana, geneva, sans-serif;">Perform other duties as assigned</span></li> </ul> <p><span style="font-size: 12pt; font-family: verdana, geneva, sans-serif;"><strong>What You Bring and How You Qualify</strong></span><br><br><span style="font-size: 12pt; font-family: verdana, geneva, sans-serif;" data-teams="true">First and foremost, you’re passionate and committed to reimagining pediatric health care and creating a world where every child with special health care needs gets the care and support they deserve.
