FT Nurse Practitioner/Physician Assistant - Reno, NV

From private residences and long-term care to assisted living facilities, our team of nurses, physicians, advanced practice providers, and care providers visit patients where they’re most comfortable—creating a seamless, convenient, and personalized process for seniors and their caretakers.</em></p> <p><strong>The Role</strong></p> <p><span style="font-weight: 400;">The Provider is responsible for the diagnostic evaluation, management, and therapeutic health care of primary care patients of PPH in our Reno Clinic location as well as patients residing in assisted living communities, and residential care homes.&nbsp; This care includes-&nbsp;</span></p> <ul> <li style="font-weight: 400;"><span style="font-weight: 400;">Obtain history and physical on new patients&nbsp;</span></li> <li style="font-weight: 400;"><span style="font-weight: 400;">Order/interpret diagnostic tests&nbsp;</span></li> <li style="font-weight: 400;"><span style="font-weight: 400;">Diagnose and treat chronic conditions and episodic illness&nbsp;</span></li> <li style="font-weight: 400;"><span style="font-weight: 400;">Perform Advance Care Planning&nbsp;</span></li> <li style="font-weight: 400;"><span style="font-weight: 400;">Accurate and timely documentation in the EHR&nbsp;</span></li> <li style="font-weight: 400;"><span style="font-weight: 400;">Coordinates with other members of the care team, both internal and external </span><span style="font-weight: 400;">partners&nbsp;</span></li> </ul> <p><span style="font-weight: 400;">In addition to the above, the Provider is responsible for the annual comprehensive health assessments in accordance with health plan</span><span style="font-weight: 400;"> </span><span style="font-weight: 400;">partners and Medicare guidelines. The assessments may be done in the clinic or residential setting.
