<p><em>At Geriatric Specialty Care of Reno, we're expanding our model for caring for seniors that provides high-quality, comprehensive in-home healthcare services to seniors all across Northern Nevada. From private residences and long-term care to assisted living facilities, our team of nurses, physicians, advanced practice providers, and care providers visit patients where they’re most comfortable—creating a seamless, convenient, and personalized process for seniors and their caretakers.</em></p>
<p><strong>The Role</strong></p>
<p><span style="font-weight: 400;">The Provider is responsible for the diagnostic evaluation, management, and therapeutic health care of primary care patients of PPH in our Reno Clinic location as well as patients residing in assisted living communities, and residential care homes. This care includes- </span></p>
<ul>
<li style="font-weight: 400;"><span style="font-weight: 400;">Obtain history and physical on new patients </span></li>
<li style="font-weight: 400;"><span style="font-weight: 400;">Order/interpret diagnostic tests </span></li>
<li style="font-weight: 400;"><span style="font-weight: 400;">Diagnose and treat chronic conditions and episodic illness </span></li>
<li style="font-weight: 400;"><span style="font-weight: 400;">Perform Advance Care Planning </span></li>
<li style="font-weight: 400;"><span style="font-weight: 400;">Accurate and timely documentation in the EHR </span></li>
<li style="font-weight: 400;"><span style="font-weight: 400;">Coordinates with other members of the care team, both internal and external </span><span style="font-weight: 400;">partners </span></li>
</ul>
<p><span style="font-weight: 400;">In addition to the above, the Provider is responsible for the annual comprehensive health assessments in accordance with health plan</span><span style="font-weight: 400;"> </span><span style="font-weight: 400;">partners and Medicare guidelines. The assessments may be done in the clinic or residential setting. This service includes:</span></p>
<ul>
<li style="font-weight: 400;"><span style="font-weight: 400;">Conducts Health Assessment, including past medical history, review of systems, medication reconciliation, and physical exam </span></li>
<li style="font-weight: 400;"><span style="font-weight: 400;">Review and discuss the need to close care gaps and quality measures </span></li>
<li style="font-weight: 400;"><span style="font-weight: 400;">Perform/review standardized screening exams </span></li>
<li style="font-weight: 400;"><span style="font-weight: 400;">Identify all existing medical conditions </span></li>
<li style="font-weight: 400;"><span style="font-weight: 400;">Accurately and timely documentation of assessment and conditions to include quality MEAT Statements </span></li>
<li style="font-weight: 400;"><span style="font-weight: 400;">Identifies patients in need of chronic care management </span></li>
<li style="font-weight: 400;"><span style="font-weight: 400;">Identifies patients in need of pharmacy assistance </span></li>
<li style="font-weight: 400;"><span style="font-weight: 400;">Document Elements of Personal Prevention Plan and health issues to follow up with PCP </span></li>
</ul>
<p><span style="font-weight: 400;">The Provider is also responsible for ensuring a safe and smooth transition of patients following an acute or skilled stay or ED visit. The care included is- </span></p>
<ul>
<li style="font-weight: 400;"><span style="font-weight: 400;">In-home post-acute visit including history and physical and medication reconciliation </span></li>
<li style="font-weight: 400;"><span style="font-weight: 400;">Short-term management of episodic illness related to acute</span><span style="font-weight: 400;"> </span><span style="font-weight: 400;">or skilled admission </span></li>
<li style="font-weight: 400;"><span style="font-weight: 400;">Care coordination with team members for the implementation of a safe discharge plan </span></li>
<li style="font-weight: 400;"><span style="font-weight: 400;">Identify barriers/obstacles to safe, self-management in the community and work with care team to overcome or identify alternatives</span></li>
</ul>
<p><strong>What We’re Looking For</strong></p>
<ul>
<li style="font-weight: 400;"><span style="font-weight: 400;">Current Nevada NP or PA license, credentialed with Medicare, Medicaid and health plans PPH is actively contracted with (Senior Care Plus, Prominence, OPTUM,) or has the ability to be enrolled with any health plans</span><span style="font-weight: 400;">.</span></li>
<li style="font-weight: 400;">Minimum two years experience in specialty care area (Geriatrics) in private physician office, clinic setting or community-based program preferred. </li>
<li style="font-weight: 400;">Demonstrates clinical competence including strong communications skills and medical knowledge.</li>
<li style="font-weight: 400;">Remains current in licensure according to state regulation.</li>
<li style="font-weight: 400;">Positively represents the practice by communicating to all visitors, patients and staff in an enthusiastic, friendly and professional manner.</li>
<li style="font-weight: 400;">Provides a positive environment and experience with patients</li>
<li style="font-weight: 400;">Actively seeks out methods to support the practice, staff, patients and physicians</li>
<li style="font-weight: 400;">Participates in Continuing Medical Education as required by their governing board</li>

<p><em><span style="font-weight: 400;">Travel is required for this role. You must have the ability to drive between partner facilities regularly.</span></em></p>
