By partnering with rural primary care doctors, we&nbsp;reinforce the importance of trust and relationship-driven care in rural communities.&nbsp;&nbsp;We provide our partners with the staff, technology, and processes necessary to succeed in a value-based delivery model.&nbsp;</p> <p><strong>&nbsp;</strong></p> <p><strong>About the Role:&nbsp;</strong><br>Navigating the changes to our healthcare system can be intimidating and confusing for doctors and patients alike.&nbsp; The Health Navigator’s role is to make it easier. You will start by developing trusted relationships with our patients and providers and will assist with various patient-facing, administrative, and logistical tasks so that doctors can focus on delivering high-quality medical care to their patients. Navigators work for Main Street but go to work every day inside one of our partner clinics alongside clinic staff.&nbsp; They meet with and call patients, enter data into our systems, contact the pharmacy to refill prescriptions, troubleshoot challenges with insurance coverage and more.&nbsp; Main Street’s management and training teams will equip you with the training and tools you need to perform these duties.&nbsp;&nbsp; You will:</p> <p>&nbsp;</p> <ul> <li>Develop strong relationships with patients to assist them with their care&nbsp;​</li> <li>Meet with patients during the check-in process, entering data into the Main Street software platform for reporting and tracking purposes</li> <li>Call patients to schedule an office visit or remind them to pick up their medication</li> <li>Order and schedule various procedures, tests and screenings</li> <li>Identify and track progress on important gaps in patient care</li> <li>Maintain a record of patient interactions and communicate with providers using electronic health records</li> <li>Educate patients on their healthcare options, insurance benefits,&nbsp;and common medical&nbsp;conditions​</li> <li>Help patients access various community resources</li> <li>Help coordinate follow-up care after patients have been discharged from the hospital</li> <li>Demonstrate compassion for patients, adaptability, attention to detail, patience and an eagerness to collaborate with team members​</li> </ul> <p>&nbsp;</p> <p><strong>Requirements for This Role:&nbsp;</strong></p> <ul> <li>You are a self-starter who is comfortable working independently</li> <li>You enjoy meeting new people and developing relationships</li> <li>You bring a strong service mentality to your work</li> <li>You love your community and want to see it thrive</li> <li>You can skillfully explain the importance of key activities that makes patients healthier</li> <li>You are flexible and excited to tackle new challenges</li> <li>You love solving problems and will take whatever initiative is required to solve them</li> <li>You are comfortable using data to help inform decisions and activities</li> <li>You are excited by the idea of working in a fast-paced organization where change is the norm&nbsp;</li> <li>You learn and apply new information quickly</li> <li>You are familiar with and comfortable using multiple software platforms</li> <li>Work a full-time 40-hour week; Monday-Friday 8am to 5pm (1 hour lunch)</li> <li>Active unencumbered driver’s license required</li> <li>GED or High School Diploma</li> </ul> <p>&nbsp;</p> <p><em>We are an equal opportunity employer, indiscriminate of race, religion, ethnicity, national origin, citizenship, gender,&nbsp;</em><br><em>gender identity, sexual orientation, age, veteran status, disability, genetic information, or any other protected&nbsp;</em><br><em>characteristic.</em></p> <p>At Main Street, we take your privacy and security seriously.
