<h2><span class="s1"><strong>Overview</strong></span></h2>
<p class="p2">This role is employed by <span class="s1"><strong>Phoenix Med Health (MSO)</strong></span>, a healthcare management organization supporting medical practices across operations, growth, and technology.</p>
<p class="p3">You will work directly on behalf of <span class="s1"><strong>Active Vital Care (www.activevitalcare.com)</strong></span>, a mobile, multi-specialty medical provider delivering in-home care to patients across assisted living facilities, group homes, and private residences.</p>
<p class="p2">We’re hiring someone to <span class="s1"><strong>own and grow our referral network</strong></span> across assisted living facilities, group homes, home health agencies, hospice, and other healthcare partners.</p>
<p class="p4">This is a <span class="s1"><strong>growth-focused role</strong></span> responsible for driving patient volume through strong, consistent relationships.</p>
<h2><span class="s1"><strong>What You’ll Do</strong></span></h2>
<ul>
<li>Own and grow relationships across <span class="s1"><strong>facilities, home health agencies, hospice, and other referral sources</strong></span></li>
<li>Represent <span class="s1"><strong>Active Vital Care in the field</strong></span> as the primary point of contact</li>
<li><span class="s1">Drive </span><span class="s2"><strong>consistent patient referrals and new patient registrations</strong></span></li>
<li><span class="s1">Conduct </span><span class="s2"><strong>regular in-person visits</strong></span><span class="s1"> to partners</span></li>
<li>Identify and onboard <span class="s1"><strong>new referral partners</strong></span></li>
<li>Track outreach, referrals, and pipeline activity in CRM</li>
<li>Coordinate internally with scheduling, clinical, and operations teams</li>
<li>Resolve issues quickly and maintain strong partner satisfaction</li>
<li>Continuously increase <span class="s1"><strong>referral volume per account</strong></span></li>
</ul>

<h2><span class="s1"><strong>Requirements</strong></span></h2>
<ul>
<li><span class="s1">2+ years in </span><span class="s2"><strong>healthcare account management, business development, or referral management</strong></span></li>
<li>Experience working with <span class="s1"><strong>facilities, home health, hospice, or similar referral environments</strong></span></li>
<li><span class="s1">Comfortable being </span><span class="s2"><strong>in the field regularly</strong></span></li>
<li>Strong communication and relationship management skills</li>
<li>Highly organized with the ability to manage a <span class="s1"><strong>high-volume partner pipeline</strong></span></li>
</ul>
<h2><span class="s1"><strong>Nice to Have</strong></span></h2>
<ul>
<li>Existing relationships with <span class="s1"><strong>local facilities or agencies</strong></span></li>
<li><span class="s1">Experience with </span><span class="s2"><strong>CRM tools and EMRs</strong></span></li>
<li>Understanding of the <span class="s1"><strong>Medicare / senior care landscape</strong></span></li>
</ul>
