Our team approach strives to address practical, social, emotional and spiritual aspects of care, with the goal to maximize quality of life for our patients, caregivers and families.</p></div><p><strong>This full-time position offers competitive pay, generous benefits and a supportive work environment.</strong></p> <p>The <strong>Social Worker</strong> is responsible for providing medical social casework services for home care patients in accordance with compliance with organization policies and procedures and applicable laws and regulations.</p> <p><span style="text-decoration: underline;"><strong>Schedule:</strong></span></p> <p>Monday - Friday; 8:30am - 5:00pm</p> <p><span style="text-decoration: underline;"><strong>Location:</strong></span></p> <p>San Francisco &amp; Marin Counties</p> <p><em><strong>By the Bay Health follows all CDPH vaccine requirements for healthcare personnel.</strong></em><br><br><strong><u>Essential Duties &amp; Responsibilities include&nbsp;</u></strong>(but are not limited to):</p> <ul> <li>Evaluates the need for medical social casework services to patients in their place of residence</li> <li>Participates in ongoing interdisciplinary assessment of the patient</li> <li>Assesses social and emotional factors relating to the patient's condition to estimate the capacity of the patient and family to cope with problems of daily living</li> <li>Participates in ongoing interdisciplinary assessment of the patient</li> <li>Accurately assesses the need for, and performing, revisits</li> <li>Accurately assesses the need for and initiating referrals to community resources</li> </ul> <p><strong><u>Education/Experience</u>:</strong></p> <ul> <li>Master’s degree in social work from a school accredited by the Council on Social Work Education or Master’s degree in mental health discipline</li> <li>Minimum one-year’s social work experience in a health care setting</li> </ul> <p><u><strong>Pay Range:</strong></u><br>$44.50 - $52.30 hourly</p><div class="content-conclusion"><p>By the Bay Health is proud to serve our community as an equal employment opportunity employer.&nbsp; Everyone is valued and welcome at BTBH.
