As a <strong>Hospice Care Transitions Nurse</strong>, you will serve as a trusted clinical guide—helping patients, families, and healthcare partners understand hospice services and ensuring a seamless, supportive transition into care.&nbsp; This role blends <strong>clinical expertise, relationship-building, and education</strong>, making a meaningful difference at a pivotal time in people’s lives.</p> <p><strong>How You’ll Contribute</strong></p> <p>Reporting to the Director of Admissions, the Hospice Care Transitions Nurse plays a critical role in welcoming patients into hospice care and ensuring continuity as care transitions to the interdisciplinary team (IDT).&nbsp; This role is focused on our relationships and patients at Mass General Brigham’s Newton Wellesley Hospital and Faulkner Hospital.&nbsp;</p> <p>In this role, you will:</p> <ul> <li>Conduct <strong>comprehensive patient assessments</strong> to determine eligibility and appropriateness for hospice care</li> <li>Coordinate and complete hospice admissions, ensuring a smooth transition to ongoing care by the IDT</li> <li>Educate <strong>patients, families, and healthcare professionals</strong> across care settings about hospice philosophy, services, and benefits</li> <li>Serve as a clinical liaison to hospitals, skilled nursing facilities, physician practices, and community partners</li> <li>Proactively identify new referral opportunities and strengthen relationships with existing referral sources</li> <li>Provide educational programs and in-services to healthcare facilities</li> <li>Collaborate closely with Care Dimensions clinical teams to enhance communication, continuity, and quality of care</li> </ul> <p><strong>&nbsp;</strong></p> <p><strong>What You’ll Bring</strong></p> <ul> <li>Registered Nurse (RN), currently licensed in Massachusetts and in good standing</li> <li>Minimum of <strong>three years of professional nursing experience</strong>, with at least one year in an acute care setting preferred</li> <li>Strong assessment skills and the ability to respond compassionately to the needs of patients and families in diverse settings</li> <li>Emotional resilience and comfort supporting individuals and families during times of stress and uncertainty</li> <li>A solid understanding of hospice philosophy and end-of-life care principles</li> <li>Ability to work independently while thriving as part of an interdisciplinary team</li> <li>Valid driver’s license and reliable, insured vehicle in accordance with organizational requirements</li> </ul> <p>#INDRN</p><div class="content-pay-transparency"><div class="pay-input"><div class="description"><p></p> <p>The Pay Range listed is the anticipated hiring range of this position. &nbsp;Precise pay rates are determined through consideration of one or more of the following:&nbsp; education, training, relevant experience and work location.&nbsp; There are some cases in which the pay rate could exceed the range listed on this job post.&nbsp; Care Dimensions also offers a comprehensive benefits package – including employer-sponsored health insurance, paid time-off, tuition reimbursement, student loan paydown, supplemental insurance options and more – which adds to the company’s generous total rewards package.&nbsp;</p></div><div class="title">Pay Range</div><div class="pay-range"><span>$50</span><span class="divider">&mdash;</span><span>$57 USD</span></div></div></div><div class="content-conclusion"><p>Benefits are offered to employees that are scheduled to work 20+ hours/week, which include a generous earned time (vacation days) program, tuition reimbursement, scholarship programs, student loan paydown program, two retirement plans, in addition to medical/dental/vision/life/disability insurance, and so much more!</p> <p><span style="color: rgb(0, 114, 187);">Care Dimensions is an Equal Opportunity Employer.
