Registered Nurse (RN) – Hospice Home Visits
San Antonio, Texas | PRN or Part-Time
Pacific Hospice and Palliative Care is growing, and we are seeking a compassionate, experienced, and dependable Registered Nurse to join our dedicated hospice team serving patients throughout the San Antonio area.
This position is ideal for an RN seeking a PRN, Part-Time, or On-Call opportunity providing in-home hospice care. The ideal candidate must have a minimum of two years of hospice experience, be bilingual in English and Spanish, and reside in the San Antonio area.

Responsibilities
- Provide compassionate, patient-centered nursing care to hospice patients in their homes
- Perform comprehensive assessments, including pain and symptom management, psychosocial support, and ongoing evaluation of patient needs
- Develop and implement individualized care plans in collaboration with the interdisciplinary team, patients, and families
- Administer medications and treatments according to physician orders and hospice protocols
- Educate patients and caregivers regarding disease progression, symptom management, medications, and end-of-life care
- Provide emotional support and guidance to patients and families throughout the hospice journey
- Collaborate closely with physicians, social workers, chaplains, aides, and other team members to ensure continuity of care
- Maintain accurate and timely clinical documentation in compliance with hospice regulations and agency standards
- Participate in bereavement support and family follow-up as appropriate
- Stay current with hospice best practices through ongoing education and professional development
Qualifications
- Current and unrestricted Texas RN license
- Minimum of 2 years of hospice nursing experience required
- Strong clinical assessment and case management skills
- Experience with pain management, symptom control, and end-of-life care
- Bilingual English/Spanish preferred
- Proficient with electronic documentation systems, including Axxess or other hospice EMR platforms
- Comfortable using email, Slack, and other communication platforms
- Excellent verbal and written communication skills
- Compassionate, dependable, and team-oriented professional
- Knowledge of Medicare Conditions of Participation and Joint Commission standards preferred
- Reliable transportation and valid driver’s license required
