<p><strong>In-Home Dialysis Care Partner (1:1 Client) – Thousand Oaks, CA</strong><br><br><strong>Client: </strong>Female adult dialysis patient transitioning home Immediately<br><strong>Location:</strong> Thousand Oaks, CA (91359)<br><strong>Compensation</strong>: $25/hr<br><strong>Start Date: </strong>Urgent<br><br><strong>About This Role</strong><br>We are looking for a compassionate, dialysis-experienced Care Partner to support a single female patient who is returning home this Friday and will be continuing her dialysis treatments in the comfort of her own home. This is not an agency rotation — you will be paired with one patient, allowing you to build rapport, provide meaningful support, and become a reliable part of her weekly routine.<br><br>This role is ideal for someone who understands home dialysis equipment, values patient dignity, and wants to make a real difference in someone’s life during their transition home.<br><br><strong>What You’ll Be Doing</strong><br>• Assist with dialysis treatment setup, (NxStage or similar home machines)<br>• Ensure the environment is safe, clean, and ready for each treatment<br>• Offer comfort, emotional support, and companionship<br>• Help the patient stay on schedule with treatment times<br>• Support with light Activities of Daily Living (ADLs) as needed<br>• Monitor safety and report concerns to the coordinating nurse<br>• Maintain simple documentation of observations during your shift<br><br><strong>Who We’re Looking For</strong><br>• Someone with at least 1 year of experience working with dialysis patients (home or clinic)<br>• Preferably familiar with home dialysis systems (NxStage a plus)<br>• Reliable, calm, and supportive — someone who understands how vulnerable this transition can feel<br>• Strong communicator who can coordinate with our clinical team<br>• Must pass a background check through Herewith<br><br><strong>Why This Role Matters</strong><br>This patient deeply wants to remain in her own home rather than return to a facility.
