About the role
- Contract Type: Permanent
- Hours: 37 hours per week (Monday – Friday)
- Location: hybrid working, with a minimum of three days at the Whiteleaf Centre, Aylesbury, with frequent travel across North Bucks.
- Salary: £25,250 per annum
- Working closely with the Aylesbury CMHT the postholder will take referrals from patients across Oxford Health teams aged 18-65 for short term support and/or signpost to appropriate community support services – both within Bucks Mind and with other service providers.
- To work closely with patients to understand their recovery goals and provide ‘handholding’ support enabling individuals to attend the groups/activities in the community.
- Take referrals and support a caseload of patients through 1:1 support at the Whiteleaf Centre, seeing patients either in their own homes or in community locations, and to provide details of the range of (support) options available in the community, signposting, referring or supporting as required.
- Experience working with at risk adults with mental health needs and be able to support individuals to achieve their goals in a person-centred way.
- A positive attitude towards people with mental health challenges, in-particular people’s ability to develop and change.
- Ability to respond appropriately to people in distress, maintain appropriate professional boundaries and build positive rapport.
- Ability to prioritise work appropriately, meet deadlines, keep timely appointments and manage a varied diary.
- Ability to travel around the area with access to a vehicle for use during working hours and with appropriate travel insurance (work related mileage will be paid).
