This position offers the chance to serve an established panel of patients in a medically underserved community while working alongside in-house wraparound support, including Outreach Specialists, Care Managers, and Patient Care Coordinators.</p> <p><strong>Job Summary</strong></p> <p>As a Licensed Behavioral Health Therapist, you will provide psychosocial assessments, therapeutic counseling, and crisis intervention for patients and families. You’ll collaborate with an interdisciplinary healthcare team to ensure continuity of care, support smooth transitions, and promote quality patient outcomes.
