Here, you are not just joining a workplace, you are becoming part of a mission that truly matters.</p></div><p>The Licensed Behavioral Health Therapist- School-Based (Warren Township) will provide psychosocial assessments, therapeutic counseling, and crisis intervention for patients and families. This provider will collaborate with an interdisciplinary clinic and school team to ensure continuity of care, support smooth transitions, and promote quality patient outcomes. As a Licensed Behavioral Health professional, the provider will deliver high-quality, evidence-based mental health services to patients in a school-based and community-based setting. There is potential that providers may have to float between sites and between school districts to cover for other providers when appropriate.</p>
<p><strong>Essential Functions & Key Responsibilities</strong></p>
<p>The essential duties listed below represent the primary responsibilities of this role. Other related duties may be assigned.</p>
<ul>
<li>Conduct comprehensive psychosocial assessments and develop individualized treatment plans</li>
<li>Provide short-term therapeutic counseling to patients and families, including individual, family, and group therapy</li>
<li>Maintain accurate and timely clinical documentation in the Electronic Health Record (EHR)</li>
<li>Utilize risk screens, de-escalation, and crisis intervention techniques</li>
<li>Support patients’ emotional and behavioral needs in their school environment as needed</li>
<li>Participate in patient IEP meetings, eligibility determinations, and manifestation determination reviews</li>
<li>Collaborate with JPCHC school-based Life Skills Clinicians and psychiatry providers regarding patient treatment goals and interventions (select districts)</li>
<li>Coordinate services and support smooth transitions across levels of care</li>
<li>Participate in district- and building-level teams related to crisis response, student wellness, mental health, equity, and school climate</li>
<li>Collaborate with school personnel, including district and building administration, school social workers, school counselors, and teachers</li>
<li>Collaborate with physicians, nurse practitioners, Behavioral Health Consultants, and the broader care team at local JPCHC clinics to ensure integrated care</li>
<li>Build and maintain strong relationships with school personnel, community resources, medical teams, and protective services</li>
<li>Participate in quality improvement initiatives</li>
<li>Apply knowledge of protective service mandates for both adults and children</li>
<li>Other duties may be assigned</li>
</ul>
<p><strong>Qualifications</strong></p>
<ul>
<li>Strong knowledge of child and adolescent development, trauma-informed care, evidence-based therapeutic modalities, social determinants of health, and the challenges facing underserved and low-income populations</li>
<li>Excellent interpersonal, written, and verbal communication skills</li>
<li>Ability to pass a background check and drug screen in accordance with FQHC and regulatory requirements</li>
<li>Familiarity with special education law, IEP processes, IDEA requirements, local community resources, and social services systems preferred</li>
<li>Valid driver’s license required</li>
<li>CPI (Non-Violent Crisis Intervention) certification preferred</li>
<li>Bilingual or multilingual fluency preferred<strong> </strong></li>
</ul>
<p><strong>Education and/or Experience</strong></p>
<ul>
<li>Master’s Degree in Social Work, Mental Health Counseling, Marriage and Family Therapy, or related field required</li>
<li>Current, unrestricted, independent clinical licensure in the state of Indiana (LMHC, LCSW, LMFT) is preferred. Unrestricted associate-level clinical licensure in the state of Indiana (LMHC-A, LSW, LMFTA) is required.</li>
<li>One year experience working within a public school district preferred<strong> </strong></li>
</ul>
<p><strong>Technology Skills</strong></p>
<ul>
<li>To perform this job successfully, an individual should have moderate computer skills, including Microsoft Office (specifically Word and Excel) and other applications.</li>
<li>Working knowledge of electronic health records</li>
