Catholic Health Home Care is looking for passionate, dedicated LPNs to provide comprehensive care to patients at home.

 

Team 7: Belle Terre, Centereach, Farmingville, Head of the Harbor, Lake Grove, Nesconset, Nissequogue, Old Field, Port Jefferson, Port Jeff Station, Setauket, St. James, Stonybrook, & Terryville.

• 5x 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends

Job Details

Our Home Care LPN’s:

• Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.
• Measure vital signs, Administer IVs, change dressings, clean wounds as needed
• Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.
• Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.
• Provide patient and family education for disease management and health promotion s directed by physician’s plan of treatment.

   

   

  Minimum Qualifications:

  • New York State Licensed Practical Nurse (LPN) license
  • Minimum of one year of recent hands on experience
  • Basic Life Support Certification

Posted Salary Range

USD $33.00 - USD $40.00 /Hr.

The pay for this position ranges from $33 – 40 per hour. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
