<p><strong>Job Summary</strong></p>
<p>At VitalCaring, our team members transform lives and foster hope through genuine caring. As a Licensed Practical/Vocational Nurse, you will provide direct, compassionate nursing care to hospice patients under the supervision of a Registered Nurse. You play a critical role in managing comfort, supporting families, and ensuring every encounter reflects our values—trustworthy, capable, compassionate, proactive, and called. This role is essential to delivering dignified, high-quality end-of-life care.</p>
<p><strong>Essential Functions</strong></p>
<p><strong>Patient Care</strong></p>
<ul>
<li>Provide direct patient care in accordance with the state Nurse Practice Act, including specialized duties related to end-of-life care.</li>
<li>Implement current nursing practice following a comprehensive assessment and the established plan of care.</li>
<li>Assess and educate patients and their families/caregivers on diagnosis, care needs, and symptom management.</li>
<li>Use and prepare equipment and supplies safely and effectively, consistently adhering to aseptic technique.</li>
<li>Deliver appropriate pain and symptom management; monitor patient response to treatments and medications.</li>
</ul>
<p><strong>Communication & Coordination</strong></p>
<ul>
<li>Document all care accurately and timely in alignment with the plan of care.</li>
<li>Participate in care coordination and promptly report needs requiring involvement from other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the RN and/or Clinical Care Manager.</li>
</ul>
<p><strong>Additional Responsibilities</strong></p>
<ul>
<li>Participate in ongoing personal and professional development.</li>
<li>Contribute to quality assessment and performance improvement (QAPI) activities.</li>
<li>Perform other duties as assigned by the Registered Nurse.</li>
</ul>
<p><strong>Qualifications</strong></p>
<ul>
<li>Graduate of an accredited practical nursing or vocational nursing program.</li>
<li>Minimum of three years of nursing experience preferred; home health, community-based, or medical-surgical experience ideal.</li>
<li>Current state LPN/LVN licensure.</li>
<li>Current CPR certification.</li>
<li>Demonstrated adherence to professional nursing standards and practices.</li>
<li>Licensed driver with a reliable, insured automobile meeting organizational requirements.</li>
