Our ideal candidates have strategic prowess and the ability to use data to build best practices and implement great ideas in collaboration with our team and our community.    About the Clinic Therapist, LICSW:   Reporting to the Company’s Behavioral Health Care Manager, the Clinic Therapist, LICSW role will be a key part in creating a continued care plan for our patients through offering behavioral health services. Partnering with Providers, the Clinic Therapist, LICSW will help support behavioral health by providing basic counseling services. They will also address medication adherence barriers, connecting to community-based resources, and social determinants of health concerns by accurately documenting in our EMR (Athena). This role is expected to be a high-touch and visible role for our new and existing patients. This position is hybrid, splitting time between our Crystal and Rosedale Clinics, and remote (work from home – laptop provided).
