<p class="isSelectedEnd">Health Link Home Health and Hospice, a trusted Bay Area healthcare provider for nearly 20 years, is seeking a motivated and detail-oriented LVN Clinical Coordinator to join our Walnut Creek team. This essential on-site role supports the coordination of patient care, assists field clinicians, and helps ensure smooth communication between patients, families, physicians, and the clinical team.</p>

<h2>Key Responsibilities</h2>
<ul data-spread="false">
<li>Coordinate and support care for home health patients</li>
<li>Track, clarify, and input provider orders into the EMR system</li>
<li>Communicate effectively with clinicians, physicians, patients, and families</li>
<li>Organize and monitor medical supplies while maintaining OSHA standards</li>
<li>Assist with onboarding and orientation of new staff</li>
<li>Perform chart audits and administrative support tasks</li>
<li>Support daily clinical operations within the office</li>
</ul>
<h2>Qualifications</h2>
<ul data-spread="false">
<li>Active California LVN License (Required)</li>
<li>Minimum 1 year of nursing experience preferred</li>
<li>Home health experience preferred</li>
<li>Strong verbal and written communication skills</li>
<li>Ability to manage high-volume phone communication professionally</li>
<li>Excellent organizational and multitasking abilities</li>
<li>Proficiency with Apple devices, EMR systems, and scheduling software</li>
<li>Working knowledge of medical terminology</li>
<li>Strong customer service and patient-care mindset</li>
<li>BLS Certification preferred</li>
</ul>
<h2>Work Location</h2>
<p class="isSelectedEnd">This is a fully on-site position based in our Walnut Creek office. Remote work is not available for this role.</p>
