font-family: 'Calibri',sans-serif;">The Manager is accountable for day-to-day execution of the post-acute network strategy, driving measurable impact in length of stay, readmissions, referral timeliness, and partner reliability under Maryland’s Total Cost of Care (TCOC) and AHEAD models.</span></p><p class="MsoNormal"> </p><p class="MsoListParagraph" style="margin-left: 38.5pt"><span style="font-size: 11.0pt;
