Responsibilities
- Provide hands-on personal care with dignity and respect to our Memory Care residents
- Build meaningful relationships with residents and families Acts as manager’s designee in interactions with family members, physicians, pharmacy and other individuals.
- Collaborate with Caregivers and team members Coordinates direction of staff with manager but can make some independent value judgments during the course of the shift. Call manager as needed.
- Create a safe, welcoming environment rooted in compassion
- Provide a high level of quality customer service to Resident care residents, family members, guest and employees.
- Assess the needs of all residents during a shift, and ensures that all residents’ personal needs are met.
- Adjust shift assignments and schedule as needed to adapt to changing needs of residents.
- Assists Manager with MAR record maintenance as needed, and accuracy audits.
- Coordinates orientation and training of new employees on your shift.
- Monitor your staff performance and communicate areas of concern to manager.
- Assists Manager with quality control of documentation for your shift, verifying accuracy.
- Reports to manager any changes of condition in a resident or areas of concern during the shift in writing, in person or by phone as the situation dictates.

- The Memory Care Coordinator position a full-time position that typically works 10-12 hour shifts. The schedule will be split between two supervisory staff to have daytime coverage 7-days per week. The position will include one shift on the weekend
Qualifications
- Have obtained a high school diploma, and or GED.
- Someone who has 5 or more years of experience as a caregiver, is a Certified Caregiver, and a Medical Assistant. In addition to Memory Care experience.
- Certification in: CDP, PAC, Comfort Matters, CARES or equivalent experience implementing these types of programs
Language Skills:
- The ability to read and write in English. The ability to interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. The ability to write routine reports and correspondence. The ability to speak effectively before small groups of residents, family or staff.
Mathematical Skills:
- Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals
Reasoning Ability:
- Ability to apply common sense understanding to carry out instructions furnished in written, oral or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
Professional Expectations:
- To present oneself in an orderly manner.
- Willingness to adapt to the changing demands of this position.
- Be helpful and polite to residents, to be understanding of their needs and respect their privacy, personal property and confidentiality.
- Keep your areas of responsibility clean, neat and organized.
- Cooperate and be polite to all staff members, family and visitors.
- Knowledge of state assisted living rules and regulations.
- Knowledge of the facility policies and procedures
- Attendance at staff training/in-service meetings.
