Position: Full-Time OR Part Time Primary Care Physician
Job Type: Full-Time Schedule: Monday to Friday (No Weekends) OR Part-Time (No Weekends), Flexible Schedule
Location: San Diego Area
About Us: Aligned Geriatrics is a rapidly growing mobile medical practice that specializes in Geriatric Medicine. Our mission is to provide high-quality and compassionate care for seniors in the comfort of their homes or senior living communities. As a primary care provider with Aligned Geriatrics you will play a crucial role in improving the lives of older adults by delivering personalized healthcare services.
Physician Responsibilities:
- Conduct comprehensive primary care visits in patients' homes or senior living communities
- Collaborate with a multidisciplinary team to ensure holistic and patient-centered care
- Provide preventive care, manage chronic conditions, and address acute issues in a timely manner
- Collaborate with and coordinate care amongst various community partners to ensure continuity of care
- Determine and initiate specialty referrals when appropriate
- Order and interpret lab and imaging testing
- Prescribe medications including controlled substances when appropriate
- Build meaningful relationships with patients and their families to promote trust and understanding
- Collaborate with staff at various senior living communities to ensure timely and coordinated patient care
Qualifications:
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree
- Board-certified or board-eligible in Internal Medicine, Family Medicine, or Geriatric Medicine (preferred)
- Active and unrestricted California medical license
- Experience in Geriatric Medicine is a plus
- Strong communication and interpersonal skills
- Experience with Epic (EHR) is a plus
- Experience with Microsoft Office is a plus
- Strong time management and organizational skills
- Comfortable working independently and in team based environments
- Compassionate bedside manner with a commitment to providing patient centered care.
