PACT is an evidence-based model that provides comprehensive treatment, rehabilitation, and support to adults with severe and persistent mental illness.</p> <p>The ideal candidate would&nbsp; possess knowledge of psychiatric medications, psychiatric and medical treatment. This person would be comfortable with outreach, case management and providing support in individuals homes.&nbsp;&nbsp;</p> </td> </tr> <tr> <td> <p><strong>Responsibilities:</strong></p> <ul> <li>Act as a primary case manager to 3-6 consumers</li> <li>Provide nursing interventions to individuals</li> <li>Facilitate and coordinate medical care to include communication with health care providers</li> <li>Provide health education to individuals&nbsp;</li> <li>Assist in the management of medications and the overall functioning of the nursing department</li> <li>Formulate and regularly update Comprehensive Assessments and IAP's for clients on your caseload</li> <li>Transport consumers to work sites, meetings, daily activities, appointments, and activities as needed.</li> <li>Participate in rotating on-call coverage and evening shifts to ensure 24/7 support for clients</li> </ul> </td> </tr> <tr> <td> <p><strong>Qualifications:</strong></p> <ul> <li>Requires an associates degree in nursing</li> <li>LPN or RN nursing license</li> <li>Experience working with adults with mental illness and knowledge of psychiatric and medical medications</li> <li>Valid Massachusetts drivers license&nbsp;</li> </ul> </td> </tr> <tr> <td><strong>Schedule: </strong>Monday-Friday 8am to 4pm with rotating 12pm-8pm shift , one weekend per month, oncall on a rotating basis, holidays on a rotating basis</td> </tr> <tr> <td>Bilingual candidates fluent in a needed language may be eligible for a 6% salary increase</td> </tr> </tbody> </table><div class="content-pay-transparency"><div class="pay-input"><div class="title">Annual Salary</div><div class="pay-range"><span>$58,073.60</span><span class="divider">&mdash;</span><span>$77,680 USD</span></div></div></div><div class="content-conclusion"><p>At Eliot, we are committed to supporting you at every stage of your career, providing opportunities for growth, purpose, and community.
