More info at <a href="http://Akidolabs.com">Akidolabs.com</a>.</p></div><p>&nbsp;</p> <h2>About the Role</h2> <p>We are looking for an experienced <strong>Nurse Practitioner</strong> to join Akido’s Street Medicine team in <strong>San Francisco Bay Area</strong> and help redefine how healthcare is delivered to the most vulnerable populations. Street Medicine is rapidly becoming a specialty of its own, and at Akido you’ll be at the forefront of this change, combining hands-on care with our AI-driven tools. This is a unique opportunity for an NP who values compassion, thrives in dynamic environments, and wants to make a measurable impact beyond the walls of traditional healthcare.</p> <p>As a key member of our clinical team, you will provide high-quality, evidence-based care in ambulatory and community-based settings, working alongside physicians, nurses, and care team members to achieve optimal patient outcomes. At this time, we are only considering clinicians with prior experience; new graduates are not eligible.</p> <hr> <h2>What You’ll Do</h2> <ul> <li>Provide frontline care to patients experiencing homelessness, behavioral health challenges, serious mental illness, substance use disorders, and complex chronic conditions, including patients transitioning from incarceration.</li> <li>Manage diverse patient panels by delivering acute, chronic, and preventative care directly in the field.</li> <li>Conduct patient assessments, gather histories, perform physical exams, order and interpret diagnostics, and prescribe treatment in accordance with NP scope of practice and state regulations.</li> <li>Collaborate closely with physicians and the broader care team to address atypical and complex patient cases, while exercising independent clinical judgment within scope of practice.</li> <li>Design and implement care plans that support patients as they work toward stability and improved health outcomes.</li> <li>Collaborate daily with a multidisciplinary team — physicians, nurses, social workers, outreach staff — to prioritize and coordinate care.</li> <li>Document all encounters to maintain accurate, up-to-date records for continuity of care.</li> <li>Other duties as assigned.</li> </ul> <hr> <h2>Qualifications</h2> <h3>Who You Are:</h3> <ul> <li>2+ years of experience with and comfort working with CalAIM populations of focus (people experiencing homelessness, adults with SMI/SUD, adults transitioning from incarceration, adults with complex medical needs).</li> <li>Experience providing clinical care in street medicine or related settings (e.g., homeless shelters, outreach programs), with strong familiarity with Bay Area resources for underserved populations.</li> <li>Strong familiarity with San Francisco Bay Area resources, including medical, social, and community services for underserved populations.</li> <li>Comfortable providing direct care in outdoor, field-based settings under dynamic and unpredictable conditions.</li> <li>Possess strong interpersonal and communication skills to understand and adjust effectively to patients’ needs and the surrounding community, while working collaboratively with members of the healthcare team and the street-medicine professional network.</li> <li>Ability to travel up to 75% of work time: Valid CA Driver’s License, reliable and insured vehicle.</li> <li>Ability to maintain independent clinical judgment within scope of practice and regulations, while recognizing when to involve a physician or other specialists.</li> <li>Ability to lead, prioritize, and analyze clinical workflows, objectives, and team goals.</li> <li>Ability to direct and coordinate clinical support staff.</li> </ul> <hr> <h2>License, Certification, and Registration Requirements</h2> <ul> <li>Active, unrestricted <strong>California Registered Nurse (RN) license</strong></li> <li>Active <strong>California Nurse Practitioner certification</strong></li> <li><strong>National Nurse Practitioner board certification</strong> (AANP or ANCC)</li> <li><strong>California Furnishing Number</strong></li> <li><strong>DEA registration (or eligibility to obtain)</strong></li> <li><strong>National Provider Identifier (NPI) number</strong></li> <li><strong>Basic Life Support (BLS) certification</strong></li> </ul> <hr> <h2>Preferred Qualifications</h2> <ul> <li>Experience working with diverse and underserved populations</li> <li>Familiarity with value-based care models and team-based practice</li> <li>Bilingual abilities (Spanish or other languages)</li> <li>Bay Area residents preferred</li> </ul> <hr> <h2>Why Join Us</h2> <ul> <li>Clinical autonomy supported by a collaborative care team and advanced technology</li> <li>Competitive compensation with performance support and professional development opportunities</li> <li>Opportunity to impact the health of diverse populations in a dynamic, growth-oriented organization</li> </ul> <hr> <h2>Physical Demands</h2> <p>Most work will be in the field working with CalAIM populations (people experiencing homelessness, adults with SMI/SUD, adults transitioning from incarceration, adults with complex medical needs).
