<p>Liquid Personnel is seeking an experienced Occupational Therapist to join its client’s ACE Team based in Rhondda Cynon Taf.<br>This role is part of a progressive and forward-thinking Adult Social Care service in Rhondda Cynon Taf. The team is committed to delivering rapid-response support to vulnerable adults 365 days a year through the Stay Well@Home 2 service. As a Locum Occupational Therapist, you will play a vital role in this mission and participate in a rota covering this essential service.</p>

<ul>
<li>Provide advice, information, and signposting</li>
<li>Assess urgency of referrals and initiate enquiries</li>
<li>Safeguard vulnerable individuals and ensure accurate record-keeping</li>
<li>Transfer information to partner agencies in a timely manner</li>
</ul>
<p><strong>Assessment of Need</strong></p>
<ul>
<li>Conduct holistic, person-centred assessments</li>
<li>Collaborate with individuals and carers to agree personal outcomes</li>
<li>Explore creative solutions to maximise independence and choice</li>
<li>Arrange equipment, adaptations, and inter-agency support</li>
<li>Record unmet needs and complete documentation in line with statutory guidance</li>
<li>Attend case conferences and inter-agency meetings</li>
<li>Prepare specialist reports, including court documentation when required</li>
</ul>

<li>HCPC registration</li>
<li>Occupational Therapy Degree</li>
<li>Experience in adult social care and community-based work</li>
<li>Willingness to participate in a 365-day rota</li>
<li>Ability to work independently and flexibly</li>
<li>Access to a vehicle is desirable</li>
</ul>
<p>This is a hybrid role, with most work done remotely or in the community. Team members are encouraged to attend the office once per week to maintain collaboration and team cohesion.</p>
