About the role

Home Health Occupational Therapist (OT)

Per Diem | San Francisco or San Mateo County — Dedicated Territory Assigned

We are currently looking for a compassionate, organized, and team-oriented individual to fill the role of Occupational Therapist (OT) position for the SF & Peninsula area.

Where great OTs come to do their best work.

At Health Link Home Health & Hospice, we believe great patient care starts with supporting the clinicians who deliver it. For over 20 years, we've served the greater Bay Area with a focus on clinical quality, strong systems, and a collaborative culture.

We are hiring Occupational Therapists for Peninsula teams. You will be assigned to one dedicated territory — San Mateo County — so you can build familiarity with your patients and your area without being stretched across both.

Why OTs Choose Health Link

Dedicated Territory: Your caseload stays within a manageable radius in either SF or the Peninsula — no crossing bridges or unpredictable commutes. SF patients are spread across neighborhoods like the Sunset, Richmond, and Mission; Peninsula patients run from Daly City down through Redwood City.

SF Rideshare Support: If you take the SF territory, company-provided rideshare credits (Uber/Lyft) help cover travel to and from patient visits so you can focus on care, not logistics.

AI-Assisted Documentation: We use AI-assisted charting so you spend less time on notes and more time with patients.

Clinical Support: You are never alone. Our experienced case managers and clinical team are available in real-time to assist with complex situations or patient coordination.

Compensation & Benefits

Base Salary: $125,000 – $150,000 per year (based on experience)

Performance Bonus: Up to 25% of base salary tied to quality metrics and clinical outcomes

Total Earning Potential: $156,250 – $187,500 per year

Health: Medical, dental, vision, life, and pet insurance

Retirement: 401(k) with employer matching

Perks: Paid time off, vehicle reimbursement options, and company-provided devices (iPad)

Your Role

Provide skilled occupational therapy services focused on ADLs, adaptive equipment, and functional independence

Develop, implement, and manage individualized plans of care based on physician orders

Report changes in patient condition and collaborate with physicians, case managers, and the Director of Nursing

Maintain accurate, timely clinical records using AI-supported documentation tools

Requirements

Current, active California Occupational Therapist license (required)

Minimum of 1 year of home health experience strongly preferred

Valid California driver's license

Strong organizational skills, clinical judgment, and a compassionate communication style

Ready to Connect?

Our process is simple — a short intro call, then a conversation with our clinical team. Most candidates hear back within 48 hours of applying.
