Occupational Therapist - Integrated Triage Team

<p><span data-olk-copy-source="MessageBody">Liquid Personnel is recruiting Occupational Therapist for its client's integrated Triage Team at Bexley.<br></span></p> <p>The&nbsp;Bexley Integrated Triage Team is a multi‑disciplinary service providing initial triage, assessment, and response for adults with health and social care needs across primary and secondary care settings.&nbsp;They offer holistic, person‑centred assessments both&nbsp;in the home&nbsp;and&nbsp;over the telephone, ensuring residents receive the right intervention at the right time.&nbsp;</p> <p>This is an excellent opportunity for an OT confident in community work, holistic assessment, rehabilitation planning, and supporting adults with a range of physical and mental health needs.</p> <p><strong>What will your responsibilities be?</strong></p> <ul> <li>Deliver&nbsp;professional, personalised OT assessments&nbsp;for adults and older adults with physical disabilities and dual physical/mental health needs.</li> <li>Provide&nbsp;holistic assessments&nbsp;at home and via telephone triage.</li> <li>Recommend&nbsp;rehabilitative, preventative, and independence‑focused solutions&nbsp;to improve safety and quality of life.</li> <li>Lead on&nbsp;Access / Home visits&nbsp;and functional assessments across the borough.</li> <li>Contribute to shaping and improving the OT service through&nbsp;continuous evaluation&nbsp;and professional feedback.</li> <li>Work collaboratively with: <ul> <li>Nurses</li> <li>Social Workers</li> <li>Rehabilitation Assistants</li> <li>Physiotherapists</li> <li>Doctors</li> <li>Volunteers and wider MDT colleagues</li> </ul> </li> <li>Manage a varied caseload, ensuring timely and defensible clinical decision‑making.</li> <li>Supervise and support junior staff as required.</li> <li>Attend MDT and family meetings, offering OT expertise to guide joint planning.</li> <li>Travel independently across the borough to complete home‑based assessments (driver not mandatory unless required by service, but&nbsp;ability to travel is essential).</li> </ul> <p><strong>Qualifications &amp; Experience:</strong></p> <p>To be successful in this role you must have,&nbsp;</p> <ul> <li>Degree / Diploma in Occupational Therapy.</li> <li>Experience working with adults and older adults with&nbsp;physical disabilities&nbsp;and&nbsp;dual diagnosis.</li> <li>Strong holistic assessment and rehabilitation planning skills.</li> <li>Ability to work autonomously and collaboratively in a fast‑moving MDT environment.</li> <li>Excellent communication skills across clinical, social care, and multidisciplinary settings.</li> <li>Confidence managing a community caseload and prioritising effectively.</li> <li>Ability to travel independently to home visits across the borough.</li> </ul> <p>&nbsp;<strong>Referral Bonus:</strong>&nbsp;Know an OT who would suit this role?
