<p>Liquid Personnel is seeking an experienced Occupational Therapist to support one of its clients in meeting increased demand following the successful rollout of their Home First contract.</p>
<p>This is a high impact reablement role, focused on enabling individuals to maintain independence and reducing the need for long-term care support.</p>
</div>
<p><strong><span data-contrast="auto">What will your responsibilities be?</span></strong><span data-ccp-props="{"134233117":false,"134233118":false,"335559738":0,"335559739":0}"> </span></p>
<ul>
<li>Complete strengths-based assessments in line with Care Act principles</li>
<li>Develop reablement-focused care plans to maximise independence</li>
<li>Provide expert advice on equipment and adaptations</li>
<li>Manage a diverse community caseload, including home visits and hospital settings</li>
<li>Work closely with multi-disciplinary teams to support safe discharges and prevent admissions</li>
<li>Maintain accurate records and contribute to service outcomes</li>
</ul>

<p><strong><span data-contrast="auto">Qualifications & Experience: </span></strong><span data-ccp-props="{"134233117":false,"134233118":false,"335559738":0,"335559739":0}"> </span></p>
<ul>
<li>Qualified Occupational Therapist with HCPC registration</li>
<li>Minimum 2 years post-qualified experience</li>
<li>Experience within:
<ul>
<li>Reablement / Community OT</li>
<li>Adults Social Care</li>
</ul>
</li>
<li>Full UK driving licence (essential)</li>
</ul>
<p><strong><span data-ccp-props="{"134233117":false,"134233118":false,"335559738":0,"335559739":0}">Working Pattern:</span></strong><span data-ccp-props="{"134233117":false,"134233118":false,"335559738":0,"335559739":0}"> </span></p>
<ul>
<li>37 hours per week</li>
<li>~50% of admin completed in the office (County Hall)</li>
<li>Remaining time split between:
<ul>
<li>Home working</li>
<li>Community visits</li>
</ul>
</li>
<li>Example structure:
<ul>
<li>3 days visits</li>
<li>1 day office-based</li>
<li>1 day working from home</li>
</ul>
</li>
</ul>
<p>4-day working week considered (no less)</p>
