color: rgb(132, 63, 161);"><strong data-start="4695" data-end="4724">How You'll Make an Impact</strong></span></p> <p>&nbsp;</p> <ul> <li>Provide professional physical therapy evaluations, treatments, and interventions in accordance with physician orders and the patient's plan of care.</li> <li>Assess patients' functional abilities, limitations, and rehabilitation potential through comprehensive evaluations.</li> <li>Develop and implement individualized treatment plans designed to restore function, maximize independence, and improve quality of life.</li> <li>Monitor patient progress and adjust treatment plans as appropriate to achieve optimal outcomes.</li> <li>Accurately document evaluations, treatments, patient responses, and changes in condition in accordance with agency policies and regulatory requirements.</li> <li>Educate patients, caregivers, and family members on treatment plans, safety recommendations, home exercise programs, and the use of assistive devices.</li> <li>Collaborate with physicians, nurses, therapists, and other healthcare professionals to coordinate patient care.</li> <li>Participate in the development and ongoing revision of patient care plans.</li> <li>Supervise and provide clinical guidance to Physical Therapist Assistants (PTAs) as appropriate.</li> <li>Coordinate patient equipment needs through approved medical supply vendors.</li> <li>Identify patient and family needs and facilitate referrals to community resources and support services.</li> <li>Participate in case conferences, community education initiatives, agency in-services, and continuing education opportunities to maintain clinical excellence.</li> </ul> <p>&nbsp;</p> <p>&nbsp;</p> <p><span style="font-size: 14pt; color: rgb(132, 63, 161);"><strong>What You'll Bring to the Team</strong></span></p> <p>&nbsp;</p> <p data-start="2179" data-end="2489">&nbsp;</p> <ul> <li>Current Massachusetts Physical Therapist License in good standing</li> <li>Registered Physical Therapist</li> <li>Minimum of two (2) years of experience as a Physical Therapist</li> <li>Home health or community health experience preferred</li> <li>Strong clinical assessment, treatment, and documentation skills</li> <li>Excellent verbal and written communication skills</li> <li>Ability to build positive relationships with patients, families, and interdisciplinary team members</li> <li>Ability to work independently and manage a flexible schedule</li> <li>Strong organizational and time-management skills</li> <li>Adaptability and commitment to delivering exceptional patient care</li> </ul> <p data-start="2179" data-end="2489">&nbsp;</p> <p data-start="2179" data-end="2489"><span style="font-size: 14pt;
