This may include assessing and evaluating therapeutic/rehabilitative/functional status and participation in developing the total plan of care as well as providing direct physical therapy treatment.<strong><br></strong></li> <li>Assess patients for muscle strength, mobility, gait, ROM and transfer capabilities.</li> <li>Instruct patients, families and caregivers in the use and care of therapeutic appliances.</li> <li>Report patient’s reaction to treatment or changes in care to patient’s physician as needed and appropriate.</li> <li>Initiate physical therapy programs and instruct other personnel, patient and/or family/caregivers in their therapy program, as well as the goals of therapy.</li> <li>Supervise the Physical Therapist Assistant (PTA) as necessary.</li> <li>Participate in Otterbein Universal Home Health Quality Assessment/Performance Improvement program.</li> <li>Confer with the attending physician, home care team members and patient and/or family/caregivers to promote coordinated and comprehensive care.</li> <li>Prepare and submit clinical and progress notes in a timely manner and in accordance with Otterbein Universal Home Health, state and federal guidelines and regulations.</li> </ul> <p><strong>Education and Experience</strong></p> <ul> <li>Education: Bachelor’s degree or master’s in physical therapy.</li> <li>Experience: Minimum 2 years of physical therapist experience with community / home health preferred.</li> <li>Licensure, Registration, and/or Certification: Current license as a Physical Therapist from the State of Ohio or made application for same. Must have a valid Ohio driver’s license and access to reliable transportation.
