<p>InHome Therapy is hiring <strong>full-time</strong> licensed home care Physical Therapist (PT) to serve adult and geriatric patients in their homes in and around <strong>Hoffman Estates</strong>. <li>IHT PT Clinical Excellence Bonus - earn additional weekly pay based on visit complexity and volume</li>
<li>The evaluation visit rate for this role ranges from $60 - $95 per visit</li>

<p><strong>Key Responsibilities</strong></p>
<ul>
<li>Deliver in-home physical therapy services to adult and geriatric patients in your community</li>
<li>Evaluate patient mobility, strength, and function to develop individualized treatment goals</li>
<li>Implement effective rehabilitation and therapeutic exercise programs</li>
<li>Educate patients and families on recovery plans and preventive care</li>
<li>Maintain accurate and timely home health care documentation per Medicare and agency standards</li>
</ul>

<p><strong>Requirements</strong></p>
<ul>
<li>Active physical therapy (PT) license in good standing</li>
<li>BLS CPR certification</li>
<li>Reliable transportation to see patients in their homes</li>
<li>Basic comfort with technology - email, texting, EMR systems (training provided)</li>

<p><em>*This position is paid on a per-visit basis. Final compensation is based on experience, qualifications, and internal equity. Benefits eligibility varies based on employment status and weekly hours worked. Some full-time roles are eligible for medical benefits only; others are eligible for both medical benefits and PTO. Additional offerings may include 401(k), life and disability coverage, voluntary supplemental insurance options, and a company-paid Employee Assistance Program (EAP).</em></p>
