What You'll Do
- Evaluate, diagnose, and treat a broad range of adult medical conditions, including chronic illnesses such as diabetes, hypertension, COPD, and heart disease.
- Conduct routine and preventive care visits, screenings, and wellness checks to promote long-term health and early detection of disease.
- Order, interpret, and follow up on diagnostic tests and lab work.
- Develop evidence-based, personalized care plans in collaboration with patients and their families.
- Manage a continuity panel of adult patients, ensuring appropriate follow-up and long-term care.
- Document patient encounters, diagnoses, and treatment plans accurately and promptly in the electronic health record (EHR) system.
- Coordinate care with specialists, behavioral health professionals, and other providers as needed.
- Participate in quality improvement efforts and initiatives tied to clinical performance metrics and value-based care goals.
- Deliver culturally sensitive and linguistically appropriate care tailored to Houston’s diverse community.
Qualifications
- Completion of an accredited Internal Medicine residency program
- Board Certified or Board Eligible in Internal Medicine
- Unrestricted and active Texas Medical License (M.D. or D.O.)
- Current DEA registration (Texas)
- Current BLS and ACLS certifications
- Proficiency in Spanish or another second language is a strong plus
- Experience in a value-based care or community clinic setting is preferred
- Excellent clinical judgment, communication, and teamwork skills
- Passion for delivering high-quality, patient-centered care to underserved populations
- Bilingual Spanish preferred

- This position is located in-office. The clinic address is 509 W. Tidwell, Suite 200, Houston, TX 77091
- The national target base salary range for this role is: $200,000 - $245,000. This salary range represents our national target range for this role. Actual compensation will be determined based on geographic location (current or future), experience, and other job-related factors.
- Sign-on bonus and annual performance bonus structure aligned with value-based care outcomes, including quality metrics, patient engagement, and panel management performance
- Astrana employees who refer additional physicians to our practices will be eligible to receive a $3000 bonus per physician hired OR $3500 bonus per bilingual physician hired!
