Lead Transformative Mental Health Care in Your Community
Assertive Community Treatment Team Manager - Young Adult
Location: 2 Old County Road, Barrington, RI
Schedule: Full-time, community-based role (not hybrid eligible)
Reports To: Director of Community Support Services
Department: Behavioral Health
This position requires community travel, on-call responsibilities, and carrying a reduced clinical caseload.

Key Responsibilities
- Lead comprehensive care coordination that integrates physical health, behavioral health, and social determinants
- Reduce hospitalizations and emergency interventions through proactive, evidence-based treatment planning
- Improve client stability, housing retention, and community integration through coordinated support
- Collaborate with hospitals, courts, law enforcement, primary care providers, and community agencies
- Create seamless care transitions that keep clients connected to the services they need
- Strengthen EBCAP's reputation as a leader in community-based mental health care
- Mentor and coach a multidisciplinary team through ongoing supervision and competency development
- Implement evidence-informed practices that reflect the latest in ACT model fidelity
- Foster a culture of continuous improvement, accountability, and compassionate care delivery
- Maintain Joint Commission standards and state regulatory compliance
- Monitor performance metrics that demonstrate program effectiveness
- Lead quality improvement initiatives that enhance service delivery and client outcomes

Qualifications
Required Qualifications:
- Master's Degree in human services, Bachelor's Degree in human services with a LCDP or RN license
- 3-5 years post-graduate clinical experience in rehabilitative community-based mental health
- 1-2 years of managerial and supervisory experience
- 2 or more years of experience working with adolescents and young adults in a community setting
- Experience serving individuals with Severe and Persistent Mental Illness
- Knowledge of co-occurring disorders and Medication-Assisted Treatment (MAT)
- Familiarity with BHDDH regulations
