<p><span data-contrast="none">The Psychiatric Care Coordinator / Nurse (RN) plays a key role in supporting behavioral health services through clinical care coordination, patient follow-up, and interdisciplinary collaboration. This position focuses on ensuring continuity of psychiatric care, supporting patients across levels of treatment, and reducing gaps in care through proactive outreach and coordination. The Psych RN will also assist with clinical workflows, patient education, and administrative support to enhance the efficiency and quality of psychiatric services. In addition, this role will be responsible for medical case management of PCMH target populations whose needs often bridge psychiatric and physical medicine.</span><span data-ccp-props="{"201341983":0,"335559685":10,"335559738":240,"335559739":240,"335559740":266,"335559991":10}"> The schedule for this role will be 8 AM-4 PM, Monday through Friday.</span></p>
